Main duties of the job

The successful candidate will have a flexible approach and be a team player who is able to forge excellent working relationships with multi-disciplinary teams and partner agencies. The applicant will work within a range of relevant principles including Recovery and Personalisation; empowering individuals to take control through personalised, strengths and asset based models of assessment and intervention and putting them at the centre of everything we do, recognising the rights of all individuals to gain access to valued roles, relationships, activities and facilities within their local community.

The role will involve assessing; accessing; and implementing a range of social care services and providing guidance/advice and information for adults with acute or long-term physical health, mental health needs and for older people and carers. The post holder will be supported to develop within this role and can access training and development within MPFT. The post holder will be part of a team of professionals consisting of Social Workers, Care Act Assessors, Care Act Assistants, Occupational Therapists and Physiotherapists. Working closely with MPFT Home First service and external partner agencies who provide the Reablement care.

The post holder will be expected to work between 08:30 - 17:00 and have a full driving licence and access to a car. There are options for flexible working hours and this can be discussed.
