Job summary

We are offering an exciting opportunity for a Band 5 NMC Registered Nurse to join our Out of Hours Community Nursing team in the North/Central/South Staffordshire Divisions of our Trust. We have vacancy available in all areas.

We are looking for a flexible, dedicated, and compassionate nurse to further enhance the high-quality patient experience already offered by our community teams.

You need to have a commitment to providing care in patients own homes, be able to work on your own initiative and work well within a team demonstrating a wide-range of clinical skills. Community services are admitting patients with a more complex acuity than ever before with increasingly specialist treatment requiring care from highly skilled staff.

You will also have an opportunity to undertake training and development to ensure you are well equipped for the role, with opportunity to advance further. Extensive in house support is available from the teams own clinical lead.

The Out of Hours District Nursing service offers flexible working and shift patterns to suit many different lifestyles. The shifts cover 7 days per week evenings and nights to fully embrace the ethos and values of patient centred care. The team are friendly and supportive, with a key focus on staff wellbeing and safety.

Due to the nature of the role and the geographical area covered you will require a valid driving licence and access to a car.

Main duties of the job

As a Band 5 Nurse you will responsible for the delivery of care to patients who are in their own homes or residential homes, and or delivery of care to patients within a clinic setting where these are available and appropriate. You will provide assessment of care/support needs and in the development, implementation and evaluation of care/support packages.

The care/support plans may include the following:

Promoting independence/self-management and maintaining safety

Increased functional ability

Improvement in health and wellbeing including mental health

Interventions in disease/condition management

Prevention and reducing of health inequalities

As part of this role you will be expected to:

  • Prioritise treatment and care for individuals according to their health status and needs
  • Provide palliative and end of life symptom management
  • Catheter management and procedures
  • Wound management
  • Long term condition management
  • Prevention of hospital admission
  • Undertake Tissue Viability Risk Assessment
  • Supervisory responsibilities as required (e.g. work allocation) and may have some responsibility for service developments.
