Job summary

We have an exciting and rare opportunity to join the Community intervention service of the Southwest Alliance. The team consists of Physiotherapists, Occupational Therapists and Nurses providing intervention to adults within the South West Staffordshire locality covering from Rugeley to Wombourne across 2 team sites (Norton Canes and Perton clinic)

Referrals to the service cover a range of conditions from MSK, neuro domiciliary, frailty, acute care and more.

The team are also commissioned to support prevention of hospital admission within the ICC pathway and urgent response.

Main duties of the job

You will be required to:

Work on your own as a registered practitioner within the established multidisciplinary team, you will undertake a Community Physiotherapist role for the South West alliance Community intervention Service.

You will be expected to assess, identify, and agree goals and treatment plans using diagnosis and evidence-based treatment programmes to meet a wide variety of patient needs in the most appropriate location in the community e.g. patients' own homes, clinics, and care homes.

The role will involve supporting the team to provide training to carers, care agencies and other external partners on an ad hoc basis at the request of Operational and/or Clinical Leads.

The post holder will hold responsibility for their own caseload and will be required to work without direct supervision. They will undertake regular supervision and training of rehabilitation assistants and supervise, educate, and assess the performance of Physiotherapy students.

  • To undertake comprehensive assessments using verbal and non-verbal communication skills, highly skilled physical examination, and manual assessment techniques. Using clinical reasoning skills you will provide accurate clinical diagnosis, agree physiotherapeutic goals and the physiotherapy treatment plan with the patient/carer.
  • To work to CSP and HCPC code of professional conduct and to work within guidelines. To have a good working knowledge of national and local standards and monitor own and others quality of practice.
  • To be responsible for maintaining own competency through Continuing Professional Development activities and maintaining a portfolio which reflects personal development and meets the HCPC standards.
  • To maintain and develop current knowledge of evidence-based practice, developing specialist knowledge of conditions and patient types through in-service training, self-directed learning, tutorials, peer review and external courses.
  • To hold responsibility for own specialist caseload and be responsible for a defined area of service / patient type working without direct supervision as an autonomous practitioner.
