Midlands Partnership NHS Foundation Trust has an opportunity to join our Older Adults team in South Staffordshire.

While working with the Older Adults Community Mental Health Team, you'll shape the well-being of the elderly population in South Staffordshire; work in tandem with Staffordshire Police and Mental Health Services, embodying the Trust's commitment to people-centric care. You will leverage your extensive experience in older adult community mental health, conducting assessments, formulating person-centred care plans and managing risk assessments and offering compassionate care in highly distressing situations.

Main duties of the job

Working in the Older Adults Community Team delivering high-quality clinical service to service users and their families.

Managing a complex caseload and being responsible for clinical and caseload supervision of other team members where identified.

Operating as a care coordinator for a defined caseload of service users

Delivering therapeutic interventions, following an agreed care plan, to an identified group of people experiencing mental health-related difficulties.

You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions.

You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.

  • Active and working on the NMC register

Experience

  • Experience of service user and carer involvement working

Desirable

  • Experience of working therapeutically in community settings
