Job summary
Would you like the chance to provide crucial support to Children and Young People with a Learning Disability (LD) and/or Autism (A) (up to 25 years) at risk of admission to a mental health hospital?
As a LD/A Keyworker, you will have passion and commitment when delivering quality support within the family home, placement, or inpatient setting. You will work as part of a team to prevent - where possible - children and young people from requiring an inpatient admission or placement. We are recruiting for a full-time (37.5 hrs per week) permanent AfC Band 5, Dynamic Support Keyworker to join the Keyworker Team for children and young people with Learning Disability and/or Autism within South Staffordshire as part of Midlands Partnership University NHS Foundation Trust (MPFT). Are you an Expert by Experience or a Professional (nurse, social worker, occupational therapist, speech & language therapist, psychological therapist, assistant psychologist, or youth worker) and have experience of providing support to children and young people with a learning disability and/or autism.
Main duties of the job
- Provide information and support to CYP, their families and carers to ensure that personalisation approaches are embedded in CYP plans and that creative solutions are considered to meet the individual's needs supporting with personal health budgets and integrated personal budgets.
- Support the Lead Keyworkers, Senior Keyworkers and Strategic Lead in the delivery of the project across South Staffordshire.
- Provide support, information and signposting to CYP, their families and carers.
- Act as a single point of regular and consistent contact for a caseload of CYP.
- Work with CYP and, if applicable, their family/carer to develop a personalised plan.
- Utilise the Dynamic Support Registers (DSR) agreed Blue, Red, Amber, Green rating system to capture relevant information that will be used to support the step-down process in line with the approach within the project model.
- Working with CYP, parents, carers and people with lived experience to support the development of the Keyworker Community of Experience.
- Ensure a person-centred approach at all times, responsible for developing individualised care plans utilising specialist advice alongside the team around CYP.
- Communicate effectively with CYP, taking into consideration their cognitive developmental level, behavioural/emotional state, speech and language difficulties or psychiatric presentation. This may often require the use of more creative methods of communication including easy read, drawing, signs etc
