Job summary

County Wide Diabetes Specialist Service are looking to recruit a qualified Nurse with experience in Diabetes management for the post of Community Diabetes Nurse in South Staffordshire. The post holder will join our current team of Specialist Nurses, Advanced Diabetes Dietitian, Consultants, Educators, andAdministration staff. The aim of the service is to provide Specialist Diabetes support and interventions within the Community and across theprimary care network. You must be a qualified nurse with at least 2-3 years post registration experience. Ideally you will hold an accredited coursein Diabetes or demonstrate a keen interest in Diabetes management. The post holder will have an interest in education, teaching and mentorshipfor health care professionals and patients. This opportunity will support professional development and enhance Diabetes knowledge. Non-medicalprescribing, whilst not essential would be advantageous. Applicants must be able to work on their own as well as be part of the multi-disciplinary team. You will support nurse led clinics, multidisciplinary meetings and home visits. You will additionally play a key role in supporting andeducating patients, as well as community and practice staff to improve the quality of Diabetes care.

Main duties of the job

o Be competent as appropriate to the requirements of the role.o Always demonstrate the Partnership Trust Values.o Be a Registered First Level Nurse with current NMC Registration and have a detailed professional and clinical knowledge. They will be aspecialist practitioner and have a focus in the following areas: Diabetes care and management.o Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community andhospitals.o Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation, and evaluation of care/supportpackages.o Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and servicedevelopments/improvements.o Demonstrate initiative and be creative in finding solutions to problems.o Work with the team leader undertaking delegated tasks in team management.
