Job summary

Midlands Partnership University NHS Foundation Trust is unique in combining physical and mental health, learning disability, community and adult social care services in a single Trust across Staffordshire, Stoke-on-Trent and Shropshire.

This is an exciting opportunity to lead on the development and delivery of Mental Health Community Services to improve the safe, smooth transfer of care and use of medicines across the Mental Health setting,

The post holder will work as part of a multi-disciplinary team within the Staffordshire area and support the Chief Technician in the establishment and management of care pathways to support our community service users. This role will involve cross sector working in the community with multiple agencies to support with medication reviews for patients in a clinical setting.

The post holder will also support the Medicines Management Technical team at St George's hospital.

This role will be extremely rewarding, and requires a motivated, dynamic individual who is experienced in working with a range of stakeholders and has a track record for working autonomously, service development and delivering on agreed targets.

You will be a GPHC registered pharmacy technician with excellent interpersonal and IT skills.

Main duties of the job

Main Duties

You will provide expert advice and support on aspects of medicines optimisation to ensure the use of medicines is optimised and meets safety, statutory and governance requirement.

Act as first point of contact for any medicines management issues for the community mental health teams.

Work autonomously and be responsible for planning and delivering own workload and agreeing deadlines with Chief Technician.

Provide professional advice and support on the safe prescribing, handling and administration of medicines.

Rotate between hospital wards and community services.

To perform medicines reconciliation on admission to hospital and on discharge as necessary.

To provide medication counselling to promote self-management and independence with medication.

To participate in discharge planning.

To ensure a seamless pathway from in-patient to out-patient.

To support development of a comprehensive medicines optimisation service to the wards and clinics.

Support service development and be responsive to emerging problems escalating where necessary.

You will work to promote safe, legal, rational and economic use of medicines by all users of the pharmacy service.

The working hours are 37.5 hours per week Monday to Friday with limited Bank Holiday commitments. This provides a fantastic work life balance whilst being a valued member of the Pharmacy Medicines Optimisation Team. The post holder may be required to work flexibly to meet the needs of the service.

  • Nationally recognised Accredited Checking Technician Certificate.
  • Registered as a Pharmacy Technician with the General Pharmaceutical Council.
