Band 3 - Community Healthcare Support WorkerSouth Staffordshire - 37.5hrs a week

We are seeking to recruit an enthusiastic and highly motivated Community Healthcare Support Worker to join our South Staffordshire Community Learning Disability Team. You would cover the South Staffordshire area and we have bases in both Lichfield and Stafford.

We are an award winning service and rapidly expanding to meet the needs of people with learning disabilities in the community. This post includes 2 days working at Queens Hospital supporting the Acute Nurse Liaison with reasonable adjustments for service users attending outpatient appointments or who are admitted.3 days a week will be supporting service users in the community and may include those that are attending outpatient appointments at Queens Hospital or County Hospital.

This is an exciting opportunity for anyone that has a passion for supporting people with learning disabilities in the community.Previous experience of working with people with learning disabilities in the community is essential.

The post holder will need to be able to meet the travelling requirements of the post and access to a car is essential.

Main duties of the job

You would work as part of a multi-professional team consisting of Nurses, Occupational Therapists, Physiotherapists, Psychiatrists, Psychologists and Speech and Language Therapists.

You will work collaboratively with the Specialist Community Learning Disability Team, mainstream services and statutory and Independent Sector Services.

You will visit clients in their own homes, in day services, in residential care homes or in other community settings.

KEY RESPONSIBILITIES1. To carry out clinical activities as requested by health care professionals within the community team. All clinical work will be agreed with, planned (based on a clinical formulation) and executed under the supervision of the appropriate health care professional. All clinical activities to be documented clearly in the care plan with supervision provided by the health care professional responsible for the care plan.2. To work in Queens Hospital 2 days a week supporting reasonable adjustments to be put in place for service users attending the hospital. Where required support service users with hospital appointments at County Hospital, Stafford and Queens Hospital, Burton.4. To work with service users who have very complex needs which may include severe learning disabilities, autism or additional mental illness.5. To contribute to the overall assessment, planning, implementation and review of individual service users and to lead specific assessments under the supervision of qualified staff.6. To be part of a person centred service where the needs of the individual will determine the type of intervention. The post holder will work across a variety of settings which are relevant to the service user.7. To carry out work in the community sometimes in conjunction with other members of the multi-disciplinary team.8. To complete physical health investigation tasks such as pulse, blood pressure monitoring, temperature, ECGs and venepuncture.9. To work as part of a team and in conjunction with other professionals, statutory agencies and any associated service providers for the service user. This may include the Intensive Support Team.10. To develop a professional relationship conducive to the client reaching their full potential, within stated goals.11.
