Senior Community Mental Health Nurse

Secondment - 37.5hrs - Mon to Fri - until March 2025

An exciting secondment opportunity has arisen for a Senior Community Mental Health Nurse to join the Assessment and Diagnosis (A&D) team within the Shropshire, Telford & Wrekin Dementia Assessment and Support Service (DASS).

The successful applicant will need to have experience of comprehensive assessments of patients with a suspected dementia presentation; and also being able to understand and support the needs of the carer, in this context.

This role will be working within the A&D function of the team, and will predominantly be undertaking new assessments in care home settings. The successful applicant will work alongside, and be supervised by experienced colleagues from different professional groups to contribute to the formulation of diagnosis and consider treatment planning. This will be for care homes who refer to us county wide. The post holder will also have an opportunity to take part in the Diadem project, which the service is undertaking.

The successful applicant will be highly motivated, work part of a team and will work holistically to improve the quality of life for people with dementia. This is an exciting opportunity for professional development, and we welcome informal discussions.

It is essential that you have a full UK driving licence and the use of a car.
