As part of an expanding workforce working with rough sleepers an exciting opportunity has arisen to support people with their mental health who are rough sleeping, in temporary accommodation or at risk of losing their accommodation.

The rough sleeping team continues to grow and develop, this is an exciting opportunity to be a part of a team which you can have input on the development, expansion and service delivery. Working and supporting some of the most vulnerable to ensure that everyone in society has the access to the services they require. The role itself is challenging and changeable but ultimately very rewarding.

The team delivers services to the population of Shropshire, Telford and Wrekin. The base for the post is flexible and can be negotiated with the service manager. A level of flexible and remote working is supported.

Main duties of the job

The post holder will provide expert clinical knowledge and interventions, advice, support and to the rough sleeper task force. To undertake rapid assessment and support to adults 18+ who are homeless or rough sleeping. To provide specialist mental health assessment and support to adults who are housed and supported by the local authority but whose tenancy is in jeopardy due to a decline in their mental health. To participate in the rough sleeper task force/mental health alliance/RESET multi-agency meetings, to participate in multidisciplinary, multi-agency meetings including but not limited to the HISU and Blue light project. To take a lead role to support the development of Rough Sleepers task force within Shropshire Local Authority. The post holder will be responsible for providing leadership to deliver evidence based care and interventions and providing clinical leadership within the pathway team.
