Title: District Nurse
Salary: Band 6
Hours: 37.5 Hours / Week (FULL TIME)

A vacancy has arisen for an enthusiastic individual to join our District Nursing Team, providing care to clients in their own homes.
You should have excellent communication and interpersonal skills and the ability to organise and prioritise your workload without direct supervision. Along with other Band 6 colleagues, you will support the Operational Manager in managing and coordinating aspects of care and service provision including case management within the team.
Past experience within a community setting is essential. You must have excellent leadership skills and possess the Special Practice Qualification. You will be supported by the Lead Specialist Practitioner and Operational Manager and through peer support from other Band 6 District Nurses within the team. There will opportunities for personal development through informal and formal learning, including the opportunity to complete V300 Prescribing Course if not already achieved.

Main duties of the job
You will be part of the District Nursing Team that covers Tamworth. The service provides treatments and assessments to housebound patients living within our community.
As a Band 6 within the team, you will be responsible for allocating planned visits, triage new referrals received from our Local Access Point and be a leader within the team in order to support our Band 5's, 4's, 3's and 2's.
We are a proactive team, encouraging self care to those we visit and you will also manage a caseload ensuring that holistic assessments are robust, and that patients are managed appropriately, accessing our ambulatory services when they are able to.
