The role of Access Navigator is to supervise Assistant Access Navigators, and support the Mental Health Practitioners in the MH Hub Team to ensure a high quality service is provided to all service users.

Main duties of the job

  • Provide appropriate assistance and support to people who contact the service and their carers. This will include supporting individuals to access health, social care, community services and mainstream community life
  • Complete an assessment with people while considering their financial and other resources, enable them to access support services which will assist with their health and social care needs.
  • Contribute to and monitor risk assessments and risk management plans, using a traffic light system in consultation with appropriately qualified team members in line with Trust Procedures, involving other professionals as required
  • A commitment to delivering a flexible service which operates 24 hours a day, 365 days a year
  • To supervise and assist the Assistant Access Navigators on the Access Team to ensure a quality service is provided.
