Mental Health Support Time and Recovery Worker (Hospital Discharge Team)

We are looking to recruit an enthusiastic Band 4 Mental Health Support Time and Recovery Worker, to work in our developing Hospital Discharge Team.

The Hospital Discharge Team are a multi-disciplinary team that provides mental health services to people who have been recently discharged from our Mental Health Inpatient Wards. We follow a "person centred" model of service delivery focusing on the direct needs of the service user, working across boundaries of care, organisation and role.

You would provide Support & give Time to an allocated group of service users with more complex needs and thus promote their hope and recovery, and facilitate social inclusion within their normal community environment.

Main base to be negotiated; however, work will involve travel over South Staffordshire (Stafford, Seisdon, Cannock, Lichfield, Tamworth, Burton)

Main duties of the job

  1. To be an integral member of the Discharge team, working with individuals who are about to be discharged from Mental Health Inpatients delivering interventions under the instruction/supervision of qualified Mental Health professionals.
  2. To engage the identified individuals and to provide support and interventions to meet their health and social needs through individual work.
  3. Support and work with the team to improve the interface between services for people with mental health difficulties, such as housing/financial wellbeing.
  4. Work with the Mental Health Practitioners in the team to provide tailored individual care and support plans describing anticipated treatment/s and support packages that promote recovery.
  5. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
