Community Rapid Intervention Service Advanced Clinical Practitioner Band 8a - as part of the Acute Care at Home Service

The ACAH Service is looking to recruit highly motivated and enthusiastic qualified Advanced Care Practitioners (ACP) to join the Community Rapid Intervention Service (CRIS) who have a passion for providing acute medical care to patients in their own home to avoid acute hospital admission.

The ACP will provide direct clinical care, working as an autonomous practitioner, with the aim to enable patients to remain in their own homes by providing a 2-hour urgent response to support avoidable Accident and Emergency attendances. The ACPs assess, investigate, diagnose, treat, manage and refer to community services.

The Acute Care Home Service is a collaborative partnership across Staffordshire, with MPFT, UHDB and UHNM, who fully support the NHSE, former Health Education England, Multi professional framework which is mirrored in trust policy and strategy. Therefore, consideration of applications from individuals who meet the criteria for trainee ACP are also welcome and will be fully supported to achieve academic and clinical competence of an ACP with the speciality of acute care at home.

Main duties of the job

  • As a CRIS ACP, you will work alongside the Clinical Lead ACPs & the operational management team strategically. Each qualified ACP will have a job plan which will reflect all four pillars of advanced practice & will enable each ACP to contribute to the planning and development of integrated services, act as a driver for change and ensure quality, evidence-based care to our service users. Also, opportunities to provide education to the whole multi-disciplinary team within the ACAH service to ensure continuous professional development.
  • Advanced knowledge, skills and judgement for high level and complex decision making in an extended scope of practice
  • Completing a full holistic health assessment using a range of assessment methods whilst working with patients, relatives and carers
  • Request imaging and investigations to underpin and support clinical decisions and diagnosis
  • Make independent diagnostic decisions based on an expert clinical assessment and investigation interpretation when appropriate
  • Make complex clinical decisions based on sound evidence base practice and clinical expertise
  • Initiate, rationalise and suspend prescribed treatments and medication, therapies, lifestyle advice and care
  • Carry out invasive and non-invasive diagnostic and therapeutic procedures where appropriate
  • Refer to other health professionals and services as appropriate
  • Discharge patients from the service as required
  • Maintain meticulous evidence of clinical specific competencies of advanced skills

  • oCurrent Registration with professional body: NMC or HCPC
  • oMaster's Degree in Advanced Clinical Practice
  • oProfessional portfolio evidencing the four pillars of advanced practice and clinical competence.
