Triage Nurse - Community Nursing

Job summary

Using nursing experience to provide remote triage of calls and referrals made to the community nursing teams in North Staffordshire. Liaison with patients; carers and service providers to support clinical decision making. Making appropriate decisions following comprehensive remote triage (using available information from referral information and electronic clinical systems) to ensure the safe management, and allocation visits where needed, in response to calls and referrals received by the community nursing service.

Main duties of the job

  • Work as a triage staff nurse with a professional and clinical knowledge of community nursing. Whilst not providing regular face to face patient contact the post holder will need to maintain an up to date knowledge of the clinical aspects of community nursing in order to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. This includes palliative care (particularly symptom management and support to families in the last few days of life); wound care; medication management; diabetes management; catheter and bowel care.

Main Duties and Responsibilities

  • To adhere to and apply the Nursing & Midwifery Council (NMC) code, and all other relevant guidance from the NMC. To be professionally accountable and responsible for all aspects of own work.

  • To establish and maintain effective working relationships both within the Trust and with external organisations in order to meet service requirements.

  • To determine eligibility of patients referred to the service and collect additional information to support decision making. To transfer or redirect referrals to appropriate internal or external services as appropriate.

  • Liaise with patients; families; carers and other service providers to support clinical decision making to identify appropriate management of referrals and messages. This may include providing advice on self-management; transfer to other services or adding contacts for home visits or clinic appointments.

  • Liaise with nurse in charge of the appropriate nursing team to allocate urgent visits and to discuss potentially complex referrals.

  • To participate, as a member of the wider LAP team, in the development and review of policies; procedures and clinical protocols relating to the LAP service.

  • To participate, as a member of the LAP team, in service development. Providing feedback on proposed and ongoing changes and to reflect on own work to suggest areas for improvement and innovation.

  • To review all referrals and messages to the community nursing service, this will include an understanding of the following areas of care:

  • Symptom management of deteriorating patients.
  • Palliative and end of life care.
  • Wound care.
  • Bladder catheter and bowel care.
  • Recognition of deteriorating patients and potentially life threatening conditions.
  • Complex patients with co-morbidities.
  • Increase functional ability.
  • Improvement in physical and mental health and well-being.
  • Interventions in disease and condition management.
  • Admission avoidance.
  • Supporting discharge.

To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy.

To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.

To maintain contemporaneous and accurate records as required by the Trust and in line with professional standards.
