Job summary

This post will work in the Stafford Clinical and Assessment Team (CAT) and provide clinical and managerial leadership to the team consisting of qualified and unqualified OT and Physio Staff.

The team provide short term recovery, rehab and reablement to those on the Discharge to assess (D2A) pathway who have been discharged home following an acute episode and hospital admission.

The team work closely with an in house and external agency in devising and reviewing rehab programmes. The also works closing with the attached. social work team when individuals require long term therapy input.

This post holder will have both a caseload and support the Operational Lead in managing clinical staff

Main duties of the job

The Post holder will:

  • Clinically lead reablement D2A teams and manage a complex Physiotherapy/Occupational Therapy caseload.
    • Be competent as appropriate to the requirements of the

    • Be a Clinical Lead Physiotherapist/Occupational Therapist and have a detailed professional and clinical The post holder will be an advanced clinical lead practitioner and have a focus on reablement and rehabilitation.
    • Be responsible for a defined population of service users in a variety of settings across the integrated care system including the community and
    • Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support
    • Have supervisory, training and leadership responsibilities where appropriate and will have responsibility for team performance and service developments/improvements.
    • Demonstrate initiative and provide a solution focused approach to challenges.
    • Work with the operational lead undertaking delegated team management/leadership activities.

To assess new service users and develop clear individualised reablement care and support plans to meet identified needs. This includes assessing service users with specialist knowledge and skills, formulating, developing, implementing and evaluating care plans. This role would be expected to take a lead in specialist and complex cases and to provide advice and support to others who may be working with the same caseload.

To maintain sensitivity at all times to the emotional needs of the service user and their carers/family offering reassurance and empathy, in particular when imparting

potentially distressing information. This may be in regard to the nature/prognosis of their condition and implications of the same including managing any conflict that may arise.

To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that requires managing.
