We are seeking a highly motivated individual with excellent interpersonal and communication skills who has robust community nursing experience, who wishes to develop their clinical skills and knowledge further within this specialist LTC service. You must be committed to supporting patients with a diagnosis of Heart Failure utilising newly learnt knowledge, to provide holistic support of this patient group.
Essential elements of the role include assessing, planning and the implementation of holistic packages of care to support patients and their carer's who have a diagnosis of Heart Failure. The main aim of the specialist role is to provide an intense level of care to optimise therapies, support and educate the individual to manage their cardiac condition, preventing unnecessary admissions to hospital and to improve the provision of palliative care.

Main duties of the job
You will need to be an NMC registered adult nurse with a background of community nursing experience, and advantageously experience of working with patient who have long term conditions. You will need to demonstrate excellent time management and be able to adapt effectively in role, utilising newly learnt skills and knowledge to work both independently, and as a member of the Heart Failure team. A commitment to professional development, within a culture of continuous improvement is essential.

Provide an optimal level of care for patients with heart failure, by demonstrating an understanding of the underlying pathophysiology of heart failure, mechanisms of therapeutic intervention to the condition, and the significance of physical and psychological changes within the patient.
Responsible for reviewing, implementing and evaluating programmes of care, which will support the patient by providing an equitable, efficient, effective high quality and safe heart failure service.
Build upon existing partnerships with specialist palliative care services to facilitate the delivery of a palliative care approach to patients with advanced congestive cardiac failure.
Assist in the delivery of seamless care through the use of integrated care pathways in collaboration with GP's, Consultants, Palliative Care providers.
Undertake an appraisal with line manager annually
Take steps to safe guard the welfare, health and safety of all staff, patients and their families according to local policies
Act as role model in the delivery of all aspects of heart failure patient care
Responsible for clinical governance and management of risk within case load.
