Job summary

Do you want to be a GP with Extended Role supporting patients with moderate frailty. If you're interested in prevention and healthy ageing this could be the role for you!

An exciting opportunity has arisen to join the Staying Well Service, a community based multi-disciplinary team covering South Staffordshire.

We are accepting applications from GP's with an interest in frailty, prevention and healthy ageing. You'll work with an MDT to support patients to live well age well and stay well. This is an amazing opportunity if you're passionate about supporting older people living with frailty.

You will have the opportunity to work with a wide range of innovative developments such as the use of technology to support health and wellbeing. You will deliver diverse, evidence based patient care and have the opportunity and time to work with individuals over a 6 month period.

We have a strong focus on integrated working with system partners and work collaboratively across organisations. You can choose to work across some or all of the service footprint dependant on contract (East, South East and South West Staffordshire).

Main duties of the job

You will: providedevidence based, comprehensive direct patient care to patients and their carer's, taking a person centred, holistic approach.

  • To be responsible as the General Practitioner with Extended Roles (GPwER), for leading the delivery of proactive preventative medical interventions for moderately frail patients accessing the Staying Well pathway
  • contribute to the completion of a joint Comprehensive Geriatric Assessment and action plan for the individual in collaboration with the MDT and the patient.
  • assess, treat and review patients in a variety of community settings including patients own homes, community venues and clinic style consultations.
  • lead on clinical practice and the development of systems of practice to support accurate and timely interventions with a focus on preventing or delaying the progression of moderate frailty into severe frailty.
  • encourage positive and proactive health and wellbeing to promote healthy ageing.
  • gain a wide variety of experience as well as challenge and complexity which will require you to possess the relevant experience and knowledge of frailty prevention and healthy ageing required to support the medical management of the individual but will prove to be extremely rewarding to the successful applicant.
  • Provide supervision, support and advice to other professionals across the health economy and members of the MDT including the delivery of in house training and awareness to support clinical staff competency and development.

Essential

  • Current registration with the General Medical Council/ eligible for registration MRCP, MRCGP, FRCP or other recognised postgraduate qualification
  • Evidence of continuing professional development and/ or suitable experience relating to frailty, prevention and healthy ageing/ older adults
  • Up to date knowledge of all the latest guidelines and evidence in older peoples medicine
  • A working understanding of clinical governance

Desirable

  • Royal College membership
  • Mentorship and supervision qualification or equivalent experience (eg. GP Trainer, DMP)
  • Additional specialist training/ qualifications relating to frailty, prevention and healthy ageing/ older adults

Experience

  • Previous experience in primary care
  • Experience in a broad range of healthcare settings including management and clinical pharmacy activities
  • Experience of audit, service evaluation and development
