Job summary

This role aims to support the Clinical leadership of Acute Care at Home in the position of Lead ACP, alongside our current Lead ACP. This Job description and role is currently being evaluated, and will form part of a recruitment process as we move forward. We would prefer this to be a full time 37.5 hours post, but would consider 30 hours if appropriate.

The post holder will facilitate the standardisation of Advanced Practice/Advanced roles within the Acute Care at Home Service determining a Trust training programme for Advanced Practice. The post holder will be the Service lead for advanced clinical practice and will be responsible for the Service development and leadership of Advanced Clinical Practitioners (ACP). Supporting the Head of Operations and Clinical Operational Managers in developing, leading and implementing a multi-professional, organisational advanced practice strategy. They will provide specialised, expert advice, education, and clinical expertise in order to improve the patient experience.

The post holder will work alongside the ACAH senior leadership team to guide and support in the transformation of the workforce developing an A.C.P workforce to meet the ACAH service vision and objectives.

Main duties of the job

The post holder will practice as a senior clinical expert. They will work within their area of clinical expertise practicing independently and autonomously seeing undifferentiated patients with undiagnosed problems with minimal or no supervision. The post holder will maintain a clinical component to their role, this role focusing on management, leadership, strategy development and innovation.

  • Provide direct expert professional clinical care through health assessment, initiation of treatment, formulation and alteration of treatment and monitoring of care programmes
  • Facilitate the development of clinical practice through research and education of the multi-disciplinary team
  • Evaluate care through quality assurance initiatives and assessment of clinical outcomes
  • Be an expert resource for other healthcare practitioners
  • Ensure that the highest standards of clinical care are delivered
