Led by University Hospitals of North Midlands NHS Trust (UHNM) in partnership with Midlands Partnership NHS Foundation Trust (MPFT), the service has supported thousands of patients from unnecessarily attending or being admitted to hospital by providing high quality care quickly within their home. Offering specialist support via a single point of access to GPs, care homes, paramedics and community teams to refer patients with sub-acute clinical presentations to receive care at home within two hours. Providing Staffordshire's 2hr Urgent Community Response (UCR)
Supporting the NHS Long Term Plan this innovative service offers an MDT approach to supporting patients in their home providing acute care and medical management plans to those patients deemed appropriate in line with our governing criteria.
Main duties of the job
The post holder will be a Registered Healthcare Professional and hold an MSc in Advanced Clinical Practice or equivalent at MSc level to be agreed as part of the application process. They will be an independent Non-Medical Prescriber. They will practice at an advanced level demonstrating in depth knowledge and competence in all aspects of acute care within the patient's home setting & environment, encompassing enhanced assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.
The post holder will have both UHNM and MPFT responsibility to promote clinical excellence in the care of patients referred to or managed by the service, by providing clinical advice and support to nursing staff and other health care professionals. They will provide expert professional and independent clinical care which enables the coordination of a multi professional seamless service for all patients referred through the Acute Care at Home Service.
