JOB TITLE: Developmental Band 5 progressing to Band 6 Community Mental Health Practitioner:- Access Pathway (All Age)

BAND: 5 progressing to 6 after 18 months successful completion of training

HOURS: As per contracted hours to be worked during Access Team core business hours, 8am-8pm, 7 days per week inc Bank Holidays.

ACCOUNTABLE TO: Operational Manager

RESPONSIBLE FOR: Assessing mental health referrals into Secondary MH Care. Predominantly by telephone, determining MH needs, risks and Urgency of response. Delivering safe and effective services. Developing clear outcomes following triage and undertaking risk screening to agree and determine level of service to be offered to service user and carer enquiries. To provide effective advice and guidance to meet needs of service user and or professional, in accordance with Recovery Wellbeing principles. Carrying caseload of SMI clients under CPA Function.

Main duties of the job

The Band 5 Developmental MH Practitioner role is a training post where the post holder will be supported to undertake the processing, assessing and determining the mental health and social care needs of a person referred to the Service. The post holder will determine their ongoing needs and facilitation of service provision. In addition the role will require the development of excellent risk assessment and management skills in relation to and positive risk taking and decision making to enable safe and effective responses for an identified client group, largely by telephone assessment. The post holder will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved. They will assist in the delivery of a high standard of Mental Health care to service users referred to the service. They will implement agreed systems for the monitoring of effectiveness of care. They will participate in the education and management of junior staff. Upon successful completion of 18 months with the Access Team within this role the post holder will be promoted to substantive Band 6.

7. Maintaining compliance with the required range of mandatory and statutory training as well as with the requirements of registration with relevant professional bodies including: Nursing and Midwifery Council

8. To provide an active involvement in the assessment processes to ensure effective outcomes for people referred to secondary mental health services are met.

9. To triage all initial requests, including those relating to Health and Social care, eg the Care Act, (2014) Appropriate Adult, Mental Capacity (i.e. Best Interests),(2005) and Mental Health Act assessments (1983/2007)and the Human Rights Act (1998).

10. Contribute to, and facilitate the commencement of risk screening and gathering information to determine levels of need and support for service users.

11. To advise on the effects of prescribed medication for known service users and support appropriate information and guidance to service users via external sources, referring on to most appropriate agencies.

12. Ensure that the needs and views of carers, friends and family are considered (where this is deemed appropriate) and wherever possible that this is with the express consent of the service user. Ensure any appropriate carer assessments are carried out or arranged as required and referred to local authorities to offer support

13. Exchange information with other professionals in all clinical pathways and external organisations

14. To supervise and support staff in Bands 2-5 and report any concerns to the Pathway Manager or another senior team member when the Pathway Manager is absent.

15. Participate in any clinical audit as required.

16. Participate in, and where appropriate deliver, clinical supervision.

17. Participate in managerial supervision.

18. Participate in the knowledge and skills framework appraisal process.

19. Undertake an active role in the agreed induction process for new staff

20. Contribute to the annual training plan for the clinical area

21. Shift co-ordinator to coordinate the workload of the team on an as required basis.

22. Take ownership of the training requirements of the post, booking and attending training as required.

23. Contribute to the achievement of the key performance indicators for the Trust.

24. Use and populate the Trusts IT and other relevant IT systems as required. Systems and equipment

25. To work in partnership with other service providers to access and provide equipment and opportunities for service users to maximise functioning where possible. Responsible for ensuring actions are carried out where safety alerts are issued regarding equipment.
