Job summary

The South East Alliance Local Access Point is looking for an enthusiastic, organised, caring individual to join our team. LAP are a supportive, friendly team consisting of Call Handlers and Triage Nurses who support Community and Therapy services within Burntwood, Lichfield & Tamworth.

The team action email referrals and calls through a range of disciplines including self-referrals, families and from external health and social care professionals.

When you join our team, we are committed to investing in you and your career. As new starter you will receive a robust supernumerary induction plan where you will be given opportunity to shadow the teams you will be supporting in order to develop your knowledge and skill base.

You will be assigned a buddy within the team to ensure you receive support and supervision throughout your initial induction and follow on. You will have a yearly appraisal where a development plan will be agreed to aid personal and professional development.

On joining the team you will be supported to complete mandatory training and a range of clinical and IT training sessions to support your triage skills and knowledge. The candidate will not require community experience but this would be desirable.

The post does require the candidate to have a full NMC PIN Registration. Shift patterns for this post consist of 7.5 hour shifts between the hours of 8am and 18.30pm via different shift patterns covering a 7 day service including weekend and bank holidays.

Main duties of the job

Work as a triage staff nurse with a professional and clinical knowledge of community nursing. Whilst not providing regular face to face patient contact the post holder will need to maintain an up to date knowledge of the clinical aspects of community nursing in order to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. This includes palliative care (particularly symptom management and support to families in the last few days of life); wound care; medication management; diabetes management; catheter and bowel care.
