Job responsibilities

To plan and deliver an appropriate youth work programme for young people within our long term condition pathways in MPFT Targeted nursing services in partnership with UHNM, in accordance with the principles and values of youth work and the changing needs of the young people.

To lead and line manage the members in the team in line with Trust line management responsibilities and policies: ie Healthroster,

To contribute to the development of transition services and to act as an advocate and source of support for young patients at Midlands Partnership Foundation Trust.

To effectively record the achievements and outcomes of the work undertaken and present this information, where required.

To participate as a professional member of the Youth Service and contribute to the care of the patients of the Trust, relating to their all-round cultural and diversity, developmental and emotional needs.

To contribute to the development of the Youth Team through identified project aims communicating effectively with the Service Manager and Specialist Teams.

To assess the social, developmental and educational needs of individuals and groups of young people and deliver appropriate programmes and activities.

To establish relationships with key individuals and, as appropriate, parents/carers and siblings.

To establish strong relationships with appropriate professionals within the Childrens Specialist Teams and partnership organisations relevant to the role.

To develop and implement MPFT Youth Forum to create opportunities for young people to participate in service development and actively share their views within the Trust as part of co-production commitment.

To support the management of the transition of young patients between paediatric and adults units and work in partnership with the Health Specialist Teams.

To document and report any concerns relating to needs assessment and safeguarding issues, as appropriate.

To participate in multidisciplinary meetings, case conferences, reviews etc. providing professional advice on individual care programme planning, reporting any relevant information or concerns as necessary.

Ensure up to date evidence based practice by close liaison with accreditation schemes such as AQA Unit Award Scheme and ASDAN Short Courses and maintaining an awareness of the national youth work agenda.

To support young people in their community, if necessary, to help re-establish their normal routines e.g. returning to school. Enable young people to access appropriate community support services such as health, social, educational or legal services. To establish contact and relationships with groups and organisations to help establish young people in their local communities.

To undertake appropriate risk assessments to support delivery of interventions and care planning.

To assist the Service Manager in identifying changes in the needs of the service, making changes to ones own practice and offering constructive suggestions for improving services for young people in hospital.

To lead the supervision, appraisal and performance management of staff, volunteers and students within MPFT Youth Service.

To represent the Youth Service at relevant Trust meetings

To work with Service manager and appropriate corporate colleagues for finance, contracts, workforce, quality, estates etc.

To provide verbal and written reports to the Youth Service Manager; Specialist Teams and relevant charities.

Develop and implement local standard operating procedures.

Alongside other team members collect data and analyse data as defined by the Youth Service Manager.

Develop and lead project work to improve services and facilities for young people accessing hospital services.

Participation in clinical audit, health and safety, quality assurance, risk assessment, bench marking, evaluating user satisfaction, the development of clinical guidelines or protocols, Trust and departmental policies as required is undertaken.

Complete information/activity recording and data entry for the service as required.

Maintain an awareness and understanding of relevant, up to date research studies within the field of youth work.

To assist with securing external funding for the development and delivery of the Youth Service.

Attend and participate in local, regional and national youth worker hospital youth worker networks.

To participate in MPFT Children and Families Youth Service Team Meetings and training events; sharing practice and contributing to service development.

To assist the Youth Work team in delivering training for appropriate staff groups.

To provide education to healthcare professionals on the needs of young people.

To assist with the training and development of Youth Service volunteers.

To assist with the support of students and volunteers during practical placements.

To identify suitable approaches, appropriate methods and resources to support young people.

Undertake the accurate collection of data for use in monitoring the service and in audit and research projects.

Present information in reports and present information nationally at conferences and meetings.

Contribute to working with the Trusts clinical audit team and engaging in audits against NICE guidance and QSAVS.

Participate in continuing development to develop the knowledge and skills required for the post.

Participate in the Youth Service team meetings and training events.

Adhere to Trust policies.

Undertake the Trusts mandatory training

Monitor and reflect on own performance, identifying and addressing own professional needs through regular supervision sessions and appraisals.

Participate in the Trusts Personal Development Review and Knowledge and Skills Framework process and administration undertaking training identified as appropriate.

The post holder has a responsibility to keep up to date and comply with Trust policies and professional practice.

Attend departmental and Trust meetings as required.

Essential

• Youth Work/Counselling Degree or equivalent professional qualification
• JNC professional qualification
• Evidence of continued professional development.
• Substantial relevant post graduate experience
• Experience & knowledge of Youth Work Practice.
• Experience of leading a team to implement a program/pilot
• Experience of working in a multi-professional team.
• Experience of working with young people in a wide variety of settings
• Knowledge of the health and wellbeing issues affecting young people who may present in crisis.
• Experience of working with young people in a wide variety of settings.
• Experience & knowledge of Youth Work Practice.
• Experience of working in a multi-professional team.
• Knowledge, experience and application of current legislation and guidance governing the delivery of young people's services
• Excellent communication and interpersonal skills with colleagues, volunteers, other NHS staff, patients and relatives.

• Additional qualifications and/or training relating to young people's emotional health & well-being.
• Additional training or experience relating to themes faced by young people ie sexual health/ social media/ substances
• Experience of working with young people in a healthcare/hospital setting

• Knowledge of issues relating to adolescent care and transition.
• Skills and experience in teaching & training
