Senior Youth Worker - Diabetes Team

You will achieve this by creating supportive, positive, and trust-based relationships. This will enable our young people to find out what is important to them.
- You will work as part of the Diabetes team and will be responsible for supporting youth workers managing a caseload of young people aged 11 - 16 years old across various locations within the Staffordshire & Stoke On Trent area.
- This role is offered on a full time, fixed term of 2 years, working 37.5 hours per week.
Main duties of the job
- To lead the development of a youthwork model and offer supervision to youth workers and volunteers in the team.
- You will work with the clinical leaders and stakeholders to promote youthwork as an intervention for young people and present data and evidence as part of the pilot to illustrate the impact and outcomes of youthwork.
- To work as a MDT member of the Diabetes Team; to provide nonclinical support to 11- 16 year-olds living with diabetes, to promote their engagement with their diabetes, and therefore increase their health potential, their acute and long-term diabetes related health outcomes, and their wellbeing.
- To provide and lead a wide range of appropriate educational and recreational activities to enable young people aged 11 -16 years old living with diabetes to develop cognitive, social, and emotional skills on an individual or group basis, to manage their own Diabetes.
- You will be required to act as an advocate and source of support for young people.
- You will need to work flexibly and assist with the provision of a varied programme of personal, social and educational developmental activities during the day and evening, face to face, online, independently and in group settings with a wide variety of young people.
