Job summary
Health & Justice Practitioners are based within the 3 Police Custody suites based in the Black Country to offer screening and assessment. Key tasks include arranging appropriate support and liaise with the professionals within the criminal justice system, in order to assist with offering access to healthcare for vulnerable individuals, by supporting with Health and Social needs, with the aim of diverting the service user away from the criminal justice arena.
You will receive referrals from our police colleagues of individuals in Police Custody. Your role will be to check health records to aid triage and identify those who may have vulnerabilities which would meet the eligibility criteria for the Health & Justice Service. You will make onward referrals to services which are required to meet individuals needs identified from the assessment. You will also consider whether informal admission to hospital or requests for Mental Health Act assessments are required to divert people from the Criminal Justice System.
Main duties of the job
An exciting and unique opportunity has arisen for experienced practitioners with criminal justice interest to work in the Black Country's Health & Justice Service, which was formerly known as Liaison and Diversion Service. You would be expected to work in Police Custody suites based in the Black Country (Oldbury, Wolverhampton, and Bloxwich). This will include providing high quality assessments and support of people coming into contact with the criminal justice service. The unique service is delivered to people of all ages, with any vulnerability including but not limited to; mental health, physical health, learning disability, substance misuse, head injury, unemployment, housing, educational needs etc.
This exciting service is currently funded by NHS England and is delivered by Midlands Partnership NHS Foundation Trust.
The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.
Applicants must be RMN, RNLD or social worker qualified and have experience in working with service users with complex needs.

- Take responsibility for specialist assessment (within service specification) in Liaison and Diversion in conjunction with service user and carer, helping to promote self-determination, choice and autonomy.
- To provide written reports for Crown or Magistrates Courts or to attend and give verbal evidence as required. To offer considered recommendations to these bodies.
- Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
- Assess patient's individual care needs, agree and refer on to appropriate agency.
- Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
- Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition
- Utilise agreed outcome measures to review the effectiveness of process, as part of Health and Justice Service.
- Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
- Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieved.
- The post holder will be required to work within the NMC Code of Practice and professional guidelines.
