Job summary

You will be responsible for the assessment of care needs and contributing to; the formulation of the children and family's needs and the development of a relevant plan based upon the children and family's negotiated goals and relevant evidence based interventions. You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the child and family to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the child and any identified carer throughout their care.

Main duties of the job

1. Delivery of holistic assessment, intensive support and treatment for the identified community client group and within a multi-disciplinary team.2. Support for hospital admission avoidance and facilitation of, timely discharge from hospital.3. This is an extended service, 8am - 10pm, which will involve shift working, including weekends and bank holidays4. To provide an active involvement in the assessment, development, implementation and evaluation of CYP care which focuses on ensuring that CYP are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided

  • Registered Mental Health Nurse on NMC UK Register
  • BSc Degree in Nursing or another mental health related subject

Desirable

  • Psychological Intervention Training e.g. CBT, CAT, DBT
  • Phlebotomy trained

Experience

  • Experience of working with people with mental health difficulties

Desirable

  • Experience of assessing, planning , coordinating and managing people with mental health problems in a community setting
  • Experience of working as part of a team, but also to work autonomously in community settings
