Job summary

Supporting local services to deliver key components of the social carepathway including a high quality personalisation, social inclusion andwellbeing navigator service to individuals for adults in Staffordshire withacute or long term social care needs and their carers.

Providing expert leadership and governance to the personalisation andsocial inclusion service, hold line management responsibility for allNavigators and Assistant Navigators and support local services inimplementing evidence-based service delivery. Working cohesivelywith Social Care Senior Management / Divisional Management andother Social Care / Health Care staff to provide a responsive andinnovative service.

Providing information advice, support and where necessary trainingand education, to other professions within the Trust and externalagencies regarding service delivery specifics and opportunities.Ensuring service users are involved with all aspects of planning anddelivery of their own personal development plans and also in thedevelopment of the service

Main duties of the job

You will be auto enrolled into the Social Work Learning Academy membership, supporting workforce career development and aspirations, with strong leadership and support in place encouraging you to be the best that you can be. You will be fully supported to develop your skills from day one with many development opportunities available as you progress through your career including the Care Certificate, Apprenticeships in Social Care and Health and the Degree in Social Work

An exciting opportunity has arisen for a full time Team Lead to lead our new Personalisation and Social Inclusion Team for Adults with Learning Disabilities and Autism in Staffordshire. This is a new post within a new team and you will play a key role in developing and leading this service. Funding is currently secured for 12 months and we believe that with your help we can build a successful team and secure funding going forward

Staffordshire Adult Social Care teams provide an integrated, person centred, strengths focused service, to local people of working age who may be experiencing significant Social Care needs . We are supported by strong partnerships and work in collaboration with partner agencies, both statutory and non-statutory adhering to the One Staffordshire ethos which enables Staffordshire wide partnerships to support the people who live in the county.

You will become a core member of our team working closely with Senior Managers, Social Work Advanced Practitioners and Social Workers under the supervision of the Social Inclusion Pathway Service Manager. Our social care staff all work collaboratively with our Health colleagues and are valued and respected members of the Health and Social Care workforce.

You will operate within legislative frameworks, such as the Care Act 2014, Mental Health Act 1983, along with other relevant legislation and guidance, offering support based on clearly defined National eligibility criteria to adults and carers. This post is based in the Staffordshire area. We pride ourselves on providing a highly responsive, preventative and effective service to people of working age across the entirety of the County.

We would like to hear from you if you have an interest in and experience of working with people living with Learning Disabilities and/or Autism, this will include supporting people during the transition from young person to adulthood, supporting their independence within their homes, neighbourhoods and wider communities. And you can demonstrate that you are motivated, can work as part of a team, engage with people, maintain a caseload, promote independence and value people, their families and friends as essential partners

A variety of interests and skills are essential to help inspire people who use the service to move forward in their lives; the confidence and enthusiasm to work with partners to explore opportunities in local communities, support community participation and build community capacity.

On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.
