Job summary
Implement and deliver the entire diabetes education service. The post holder will be required to work without direct supervision and will support the development, implementation and evaluation of education packages in the community for people with Type 2 diabetes. This will include working closely and autonomously with the community diabetes team, GPs and practice nurses, internal colleagues within the partnership trust and other local partners / NHS trusts.
Delivery of the education will be for groups of up to 15 patients and their carers or family at community venues such as - health centres, fire stations, GP practices and supermarkets across the whole of Staffordshire. Education on the full structured education programme (DESMOND) will also be provided to GPs, practice nurses, internal colleagues and other NHS trusts as part of this service as well as reviewing opportunities to further develop the service.
Main duties of the job
- To contribute to the development and implementation of educational sessions for individuals and their carers by promoting independence and self-management.
- Communicate confidently to a group of Diabetic patients and their carers/family promoting Diabetes education and support.
- To run appointment based, drop-in group sessions or online support of diabetes education and promotion of the full structured education programme for diabetes.
- Contact patients who do not attend and encourage them to re-engage with the service.
- Where required be the first point of contact for the educational team, resolving any issues related to the team before escalation.
- To develop and run Diabetes educational sessions which will include foot care, eye care and mental health.
- Co-ordinate and manage a planned diary of educational sessions sufficient to meet the programme's targets. Work on own initiative to seek opportunities for education and raise awareness of Diabetes.
- The postholder will be required to review the effectiveness of the service (attendance, location, venue, content and accessibility of the sessions), identify and implement any required improvements as the service develops to ensure that the service meets the needs of the local population.
- The post holder will be an integral link to the Community Diabetes team providing short educational sessions on diabetes to patients and will work in a variety of settings.
