We have an exciting new role and are looking to recruit two community healthcare assistants. Your role of part of the team will be integral in supporting older adults with complex needs and long term conditions. You will be visiting people in their own homes to enhance the delivery of patient care and/or carer support.

This is an exciting role, with the opportunity to make a real difference to people.

Main duties of the job

The post-holder is responsible for the provision of planned recovery focused care working in partnership with service users.

The post-holder is responsible for working collaboratively with other Trust services to provide seamless care for service users on defined care-pathways

The post-holder is responsible for working in partnership with agencies external to the Trust supporting service user's access to services in the independent and non-statutory sector.

To provide support, and spend therapeutic time with service users with mental health needs in the community.

To support with service users and carers with social and personal care needs.

To promote service user's recovery through the delivery of high quality, outcome measured care and evidence based interventions.

  • Level 2 Diploma in Health and Social Care Or NVQ 2 in Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSE's or Functional Skills) or Demonstrable knowledge and experience as outlined below

Experience

  • Experience of working as part of a team and delivering a customer focused service.

Desirable

  • Experience of working with people with Mental Health needs as a member of a multi-disciplinary team
