Job summary

To deliver structured education for people with Type 2 diabetes, predominantly using the Diabetes Education and Self-Management for On-going and Newly Diagnosed (DESMOND) model of specialist training, as part of the Diabetes Structured Education for Diabetes Team.

The Diabetes Structured Education for Diabetes Team forms part of the trust's Long Term Conditions Team and is responsible for improving education and self-management support for people with Type 1 and Type 2 Diabetes within Staffordshire and Stoke-on-Trent.

Delivery of the education will be for groups of up to 15 patients and their carers or family at community venues such as - health centres, fire stations, GP practices and supermarkets across the whole of Staffordshire. Education on the full structured education programme (DESMOND) will also be provided to GPs, practice nurses, internal colleagues and other NHS trusts as part of this service as well as reviewing opportunities to further develop the service.

Main duties of the job

  • To directly deliver structured education for people with Diabetes.
  • To deliver the education sessions in a group session or on a one to one basis in community venues.
  • To participate in the DESMOND training and other key identified Diabetes specific courses and training.
  • Signpost those receiving education to appropriate health and other services.
  • To collate data, monitor the education provided and contribute to producing reports that demonstrate evaluation of the session and the patient experience.
  • To evaluate the teaching and the learning of the educational experience and to contribute to evolving the structure of the education service offer.
  • To co-ordinate the planning and logistics of the venues and the teaching materials required.
  • Apply communication skills to enable assessment of peoples' ability to self-care and/or optimise self-care skills.
  • Support people to make informed decisions with regard to their lifestyle choices. This will include signposting or directly advising on: driving, employment and benefits; holidays and travel; leisure activities and social habits such as alcohol intake, smoking, and managing intercurrent illness and changes in circumstances.
  • To triage referrals to ensure that appropriate people are identified and invited to attend the programme.
  • To work alongside the diabetes team and other professionals on a monthly basis to support competence and personal development.
