Out of Hours Community Staff Nurse Band 5 (Stoke on Trent and surrounding areas)

Out of Hours District Nursing team in the North Division of the Partnership Trust have an exciting opportunity for staff nurses to join the service. We are looking for flexible, dedicated, and compassionate nursing staff to further enhance the high-quality patient experience already offered.

The Out of Hours District Nursing service offers flexible working and shift patterns to suit many different lifestyles. You will work both evenings and nightshifts to fully embrace the ethos and values of patient centred care. The team are friendly and supportive, with a key focus on staff wellbeing and safety.

The successful applicant needs to have a commitment to providing care in patients own homes, be able to work on their own initiative and work well within a team demonstrating a wide-range of clinical skills. Community services are admitting patients with a more complex acuity than ever before with increasingly specialist treatment requiring care from highly skilled staff. For the right applicant, training and development will be provided to meet the needs of role, with opportunity to advance further. Extensive in house support is available from the teams own clinical lead. Due to the geographical area we cover access to a car is essential.

Main duties of the job

The Post holder will:

  • Be competent as appropriate to the requirements of the role.
  • Demonstrate at all times the Trust Values: "We put people at the heart of what we do; we deliver better health, better care, in partnership & we empower and engage our workforce to improve care & wellbeing".
  • Be a Staff Nurse and have a professional and clinical knowledge of their discipline.
  • Delivery of care to patients who are in their own homes or residential homes, and or delivery of care to patients within a clinic setting where these are available and appropriate. Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community and hospitals.
  • Be skilled in the assessment of care/support needs and in the development, implementation and evaluation of care/support packages.
  • Have supervisory responsibilities as required (e.g. work allocation) and may have some responsibility for service developments.

a.

To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy. This may be in regard to the nature/prognosis of their condition and implications of the same.
