Job summary

To offer an extended service to ensure an appropriate assessment of mental health need for children and young people in mental health crisis and in need of appropriate intensive support ensuring the delivery of safe and effective practice as part of the wellbeing and recovery action plan. You will work directly with children and young people with mental health needs and their families, carrying out assessments (including risk management) and effective treatment and/or interventions, with the aim of helping children and young people participate in normal family and community activities and to facilitate positive outcomes of care in relation to young people's mental health needs. The role will also facilitate hospital admission avoidance and seamless transitions between services where appropriate. You will participate in the education and management of junior staff.

Main duties of the job

1. Delivery of holistic assessment, intensive support and treatment for the identified community client group and within a multi-disciplinary team.2. Support for hospital admission avoidance and facilitation of, timely discharge from hospital.3. This is an extended service, 8am - 10pm, which will involve shift working, including weekends and bank holidays4. To coordinate the workload of the team on an as required basis.5. To provide an active involvement in the assessment, development, implementation and evaluation of CYP care which focuses on ensuring that CYP are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided.
