Job summary

South Integrated Community Occupational Therapy Team are looking to recruit an enthusiastic Social Care Occupational Therapy Assistant to join their busy duty team.

Our Occupational Therapy Service work with Adults with long term physical disabilities and social care needs across South Staffordshire. We support people within their own home environment, provide equipment and minor adaptations to make their home environment accessible and maintain their independence.

Occupational Therapy provides appropriate provision of means to promote people's independence in line with Care Act 2014 outcomes.

Main duties of the job

This exciting role will support our Social Care OT service by triaging and processing incoming referrals to ensure they meet the criteria and are suitable for our service. There will be an element of focussed initial assessment over the phone and provision of basic equipment where appropriate

Applicants would benefit from previous Social Care or Community Occupational Therapy experience

Knowledge and experience of electronic case record keeping would also be beneficial.

To undertake initial screenings of approaches to the service.

To undertake initial screenings of approaches to the service

To carry out initial assessments of need and prioritise the urgency of intervention under the supervision of the Occupational Therapy Team Lead.

To complete focused telephone assessments of service users needs in accordance with current legislation and anti-discriminatory practice, promoting self-directed support and ensuring demonstrable outcomes.

To hold and maintain a caseload and to accurately record needs ensuring these records are confidential but readily available to team colleagues and OT Senior Practitioner.

To include and involve other professionals and other individuals as appropriate.

Discuss with people the outcomes of their assessment, identifying their choices and options to meet their needs and to maximize their independence, considering that at all times value for money is achieved.

Write clear and concise care/ treatment plans designed to meet the identified needs of people and to ensure a range of appropriate services are in place.

To review packages, including equipment, adaptations and care as determined by trust policies as required. To notify of changes and if required to undertake reassessments.

To act as key worker where appropriate and liaise effectively.

To report major concerns to OT Team Lead.

To provide effective costings for services/ equipment identified in accordance with the trusts guidelines and current policies and procedures.

To participate in regular supervision and consultation with OT Team Lead and ensure they are made aware of all issues of major importance or concern. Where required seek help or advice from an OT which will be appropriately recorded.

To promote and implement the use of direct payments, individualized budgets and self-directed support with people needing services and their families including carers assessments.

To use risk assessments as appropriate with trust procedures.

To instruct and demonstrate use of daily living equipment.

To keep up to date with changes that affect OT within the organisation

To liaise effectively with other agencies and colleagues within the trust and act as a point of contact for providers.
