Job summary

In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs in an inclusive way. Our model places our young people's needs at the heart of what we offer to ensure that they are getting the best care possible. The Intensive Support Team offers: support and intervention for young people with high risk and complex mental health needs in an intensive time bound way. We recognise the children or young person's home or community environment as being an important part of their recovery, working to sustain the right community support, avoiding admission to hospital wherever possible.

This role as a Support Worker is a diverse one. We are looking for professionals with a passion for working with our Children & Families. You will have the opportunity to expand your skill set and have the opportunity to integrate with our core services, offering learning and development opportunities such as shadowing and specialist supervision and consultation from others working with Children, Young people & Families.

Within the CAMHS Intensive Support Team, you will be offered regular training and shadowing opportunities from across the Children & Families care group. We pride ourselves on developing team members and progressing within the team.

Main duties of the job

  • Establish and maintain relationships with children, young people, relative/carers and colleagues
  • Deliver agreed therapeutic interventions, in accordance with the care plan and as directed by the Lead Clinician/ care co ordinator or other involved professional, to assist the service user to achieve their recovery goals including; improving activities of daily living, social inclusion, meaningful occupation, accessing employment, mental health promotion and physical health promotion.
  • Undertake physical health assessments including; weight, height, blood pressure, ECG, phlebotomy.
  • Responsible for contributing towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users as outlined within their care plan
  • Regularly reports and effectively communicates patient health status and care aims in care records and verbal reports to Lead Clinician/ care coordinators and other members of the multi-disciplinary team.
  • Involvement in the review of risk, communicating any changes to the care coordinator / lead professional.
  • Maintain accurate contemporaneous records and other documentation relating to work with service users.
  • Regularly access and make use of supervision
  • At all times take responsibility for own personal and professional development

  • NVQ 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSE's or Functional Skills)
  • Health social care qualification at least equivalent to the above that equips the person with the necessary knowledge, skill and competence as outlined below

Experience

Essential

  • Knowledge and awareness of issues relating to mental health, access to mental health services, and mental health promotion
  • Experience of working with CYP with Mental Health needs

Desirable

  • Skill in undertaking physical health assessment including the measurement and recording of; weight, height, blood pressure, ECG, smoking, diet and drug and alcohol use.
