This is an exceptional opportunity to contribute to the ongoing enhancement and delivery of excellent care to our local population. As part of our team, you will collaborate with a dedicated group of SAS Doctors and ANP/ACP professionals, predominantly based at the renowned Haywood Hospital in Stoke-on-Trent.

To support your transition to our Trust, we are proud to offer a generous 'golden hello' and a comprehensive relocation package. We are committed to making Midlands Partnership University NHS Foundation Trust the ultimate workplace destination.

Notably, we have submitted plans to replace the outpatients building at Haywood Community Hospital with a cutting-edge facility worth £26.5 million. This state-of-the-art structure will provide a modern and future-proof environment, ensuring the highest standard of patient care. The new building will seamlessly integrate with the existing main hospital buildings and will continue to house our crucial out-patient services

As we strive to establish an integrated frailty service, we are seeking individuals with exceptional change management capabilities. We believe in offering a meaningful and achievable career path, allowing you to contribute significantly to this transformative initiative.

Main duties of the job

You will be a figurehead of the services based at Haywood Hospital and will provide input across the Home Based (Community) Rehabilitation and Assessment Service. The role will provide clinical leadership, mentoring and support to the established team of SAS doctors, ANP's and ACP's in delivering effective Rehabilitation and Assessment Services. These are provided in a range of settings including Home, Community Hospital Beds and specifically commissioned Care Home beds.

Join us in shaping the future of healthcare by becoming a vital part of our well-established and nationally recognised Discharge to Assess (D2A) services. We are on the brink of an exciting phase of service development and welcome innovative and flexible approaches to work. Your job plan can be tailored to foster career progression and promote subspecialty interests.

You will provide leadership for a team that delivers high quality medical care to patients on our D2A pathways, which is timely and inclusive. Supporting people to make the right choices to suit their individual circumstances and preferences.

  • To become a service of excellence through supporting teaching, training and research
  • To provide consistency and continuity of care for Haywood Hospital patients
  • To continually improve processes and pathways across all providers involved in the D2A pathways
  • To work in collaboration with other Trust clinicians including non-medical workforce
