Job summary

An exciting opportunity has arisen for a full time Band 6 Occupational Therapist to join our South Staffordshire Integrated Community Occupational Therapy Team - Adult Social Care OT

Applicants would benefit from having worked in a social care Occupational Therapy role previously or have experience of the community setting.

Main duties of the job

The Team supports adults within their own homes where the primary need is physical disability with long term health conditions. The aim of OT intervention is to assist people to remain as independent as possible in the community and is an extremely rewarding role.

Health Community OT colleagues are also a part of our team and we work closely together as an integrated OT service providing the best possible outcomes for our service group.

Knowledge of Moving and Handling people, specialist equipment, minor adaptation provision and assistive technology would be beneficial

Job description

Job responsibilities

To adhere to and apply the Health and Care Professions Councils (HCPC) standards of proficiency and the Royal College of Occupational Therapists (RCOT) code of ethics and professional conduct and all other relevant guidance from the HCPC, RCOT and MPFT. To be professionally accountable and responsible for all aspects of own work.

To establish and maintain effective working relationships both within MPFT and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values.

Working in a team and in partnership with the wider MDT you will provide specialist assessment and care planning in conjunction with service users and carers to:

  • Promote independence/self-management, increased functional ability and maintaining safety
  • Improve health and wellbeing including mental health
  • To provide disease/condition specific interventions
  • Prevent and reduce health inequalities
  • Support with admission avoidance and early supported discharge
  • Provide palliative and end of life care

To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider each service users personal circumstances and preferences in order to implement and adjust programmes of care and intervention.

To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy, in particular when imparting potentially distressing information. This may be in regard to the nature/prognosis of their condition and implications of the same.

To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.

To use professional knowledge, skills and expertise when working in changing environments and across professional boundaries.

To maintain contemporaneous and accurate records as required by the Partnership Trust and in line with professional standards (i.e. HCPC). Support identified team members giving guidance and direction where appropriate.

To take responsibility for the safe use and transportation of equipment (e.g. wheelchairs) between locations and in and out of peoples homes. This may require the post holder to work in cramped or awkward positions e.g. raising furniture (beds, chairs, toilets, therapy equipment etc.). This may be required on a regular daily basis or at least once a week.to work in non-purpose built rooms involving the necessity to move furniture, therapy equipment etc.

To ensure that the skills of identified team members are appropriately deployed and that care is delivered by staff with the relevant education, expertise and competence.

To deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role using innovative problem solving techniques.

To provide advice, education and training to service users, carers and professionals regarding the management and care/support of service users.

To provide professional and clinical support and leadership to identified team members. Supports in identifying training needs of staff and encouraging practitioners to develop skills in:

  • Critical thinking and reflective practice
  • Excellence in practice
  • Specialist knowledge and research based practice

To take accountability for allocated assessments, planning and implementation of care packages/ programmes of support, within their caseload.

Identifies health promotion needs and creates opportunities in liaison with other health care professionals to create and provide health awareness to the total service population
