Job summary
The Home First service is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands NHS Partnership Foundation Trust (MPFT).
The service seeks to offer rehabilitation programme for a time limited period to rebuild functional range of movement, confidence, relearn new skills by promoting peoples independence through health & social care interventions. The Home First Integrated service operates from 7am to 10pm over 365 days per year.
The post holder will be responsible for working in close collaboration with the Home First Multi-disciplinary Team which includes Nurses, Therapists, Care Support Workers and Social Workers.
The post holder will have exemplary communication skills, enabling them to be a role model for others, as well as positively representing the Trust values, in the delivery of effective, efficient and high quality services for service users. The post holder will be enthusiastic, passionate and a dynamic individual.
The shift patterns will be on a rotational basis over 7 days, so flexibility is required for this role.
Main duties of the job
o Be involved in the first point of contact for the discharge to assess pathway.o Assist in the delivery of outcome-based goals and self-directed goal planningo Undertake person centred support planningo Deliver support plans and safeguard vulnerable peopleo Contribute to outcome focused reviewso Encourage people's independence, choice, health & wellbeingo Identify the need and encourage individuals with the safe use of basis equipment and assistive technology, which may include in ordering basis equipment as required.o Facilitate individuals transition to alternative services and support if neededo Engage in organisational learning and developmento Promote rehabilitation and reablement individual plans of care to meet patient outcomeso Appropriate recording and completion of all written and electronic records.

Work in partnership with other health professionals and external agencies to provide a personalised service to meet individuals needs. Undertake first visits to meet CQC compliance on behalf of the Home First service as requested by the CAS Shift Lead. Read, follow and implement rehabilitation plans in accordance with the patients goals clearly documenting their progression. To monitor and report any changes to the patients individual plan of care that contributes to a continuing review of the patients care. To work unsupervised in patient's homes in the community. Promote the patient's privacy and dignity whilst undertaking rehabilitation using all personal activities of daily living for example with personal care such as bathing, showering, toileting, hygiene continence care, skin integrity, meal prep and medication. Identify and escalate any reduced patients allocated time or rehabilitation visits as evidence supports and communicate changes to management or health and social care professionals. Maintaining the patients personal information and confidentiality at all times in line with MPFTs policies. Ensure the patient is involved in all decision making regarding their rehabilitation plan working with all health and social care professionals. Promoting independence with medication, compliance and concordance enabling self-management where possible in line with Trust Home First Medication Policy. Support and educate patients in managing long term conditions to maximise their independence which will includes following rehabilitation goals set by a competent health
