- Within the Midlands Partnership University Foundation Trust (MPFT) we have a number of opportunities for Band 5 Community Staff Nurses to join our locality; this includes areas covering Lichfield and Burntwood. The positions are within the District Nursing Team, and are all full time but part time may be considered. We are recruiting dedicated and compassionate nurses to further enhance the high quality patient experience we offer in the community.
Main duties of the job
The successful applicant needs to have a commitment to providing care in patients' own homes and residential homes, undertaking Wound and Pressure Area Care, Diabetes management, Palliative and end of life care, education and support to patients and their carers. Being able to work on their own initiative and work well within a team is essential whilst demonstrating these wide-range of clinical skills. The District Nursing Team manages short and long term health needs and plays a key role in preventing hospital admissions and facilitating early hospital discharges, whilst working alongside the wider Multi-disciplinary team including GP's, our Crisis Rapid Intervention Service and the Virtual Wards.
Community services are managing patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. For the right applicant, training and development can be provided to meet the individualised needs of patients in our community.
