Main duties of the job

The post holder will work as part of a multi-disciplinary team and assist the Ward Manager in the provision of clinical leadership and be a clinical role model to the team. They will act on behalf of and deputise as directed by the manager in their absence and provide quality evidence based nursing advice, support and care for patients with mental health needs and their carers within a multidisciplinary setting.

RESPONSIBLE FOR:

  • Ensuring the effective management of a ward in the absence of the ward manager with day-to-day responsibility of managing the shift, and bed management over a 24hr period.
  • Delivery of a high standard of patient care on a daily basis and providing clinical and managerial leadership and teaching to nursing and support staff.
  • Assessment of care needs and the development, implementation and evaluation of programmes of care for patients who suffer from severe mental illness.

Job description

Job responsibilities

KEY RESPONSIBILITIES

Undertake assessments of individuals with complex mental health presentations, including those patients presenting with higher levels of risk.

Develop in collaboration with the patient their care plan and their recovery plan.

Offer specialist nursing assessment and advice where required.

Take the lead in delivering therapeutic group work programmes as required.

Administer medication, complying with relevant Trust procedures and NMC Guidance.

Ensure the safe custody of all patients as determined by the clinical team. This includes the supervision of patients within the hospital, grounds and the community, as required.

Provide clinical and managerial leadership on the ward.

Assess and manage ongoing risks as identified during assessment, ensuring that this is done in collaboration with patients and carers.

Regularly review risk factors and make changes to the management of them as necessary.

Regularly take charge of the clinical environment over a span of duty.

Work in conjunction with the ward manager in setting clinical and educational standards and monitor and maintain these standards.

Work in conjunction with the ward manager to ensure monitoring and compliance with regards to Key Performance Indicators and NHS England contractual requirement.

Regularly undertake audits of clinical activity and standards and develop plans of action to address any improvements required.

Participate in the Site Manager rota; providing out of hours (which will include night working) senior managerial / clinical advice, guidance and support on site. To be the first point of contact for fire, medical and psychiatric emergencies and coordinate Section 136 assessments as required.

The post holder will be required to work within the NMC Code of Practice and professional guidelines.

Maintain an up-to-date level of professional competence within the specific environment.

Attend meetings, multi-disciplinary ward rounds and case conferences as necessary and present information relating to patients or the service as necessary.

Provide clinical and managerial supervision to staff nurses and trainees and maintain accurate and up to date records in line with Trust policy.

Carry out appraisals for staff on an annual basis and contribute to the development of an annual training and development plan for the ward team.

With the support of the Ward Manager, undertake absence management, performance management and recruitment of staff.

In collaboration with the ward manager, ensure safer staffing levels are monitored and maintained.

Systems and equipment

Maintain timely data collection and regular inputting of information through the use of data collection systems e.g. daily diaries/RIO, Safeguard.

Responsible for ensuring that staff information is maintained in a timely manner through Health Roster Manager, including episodes of staff absence, approving annual leave requests and generation of duty rotas for the ward.

Decisions and judgements

Ensure appropriate staff resources are planned and deployed to meet safe staffing requirements through rota management using Health Roster. Adjust rotas as necessary to address, for example, episodes of staff absence and requirements for increased observations. This may involve requesting bank shifts following deployment of staff across the site on a shift by shift basis in order that safe staffing levels are maintained.

Assess patients with complex mental health, physical health and social problems to ensure appropriate treatment support and management.

Undertake a comprehensive and specialist nursing mental health assessment resulting in an individualised care plan.

Forward plan and facilitate the discharge or transfer of the patient to other services where appropriate.

Ensure that practice is evidence-based paying particular attention to the Trusts guidelines, policies, protocols and pathways.

Be available for individual supervision with agreed clinical supervisor in line with Trust policies.

Implement, review and maintain Trust Policies and Procedures and propose changes to working practices within the ward area.

Plan and organise the daily activities of the ward team in line with service need.

Communication and relationships

Communicate sensitive diagnosis and treatment related information with patients, utilising highly developed communication skills to overcome barriers to understanding.

Liaise with and advise patients and carers, local authority, Primary Care teams and other statutory bodies and third sector agencies.

Communicate sensitive information to patients, carers and relatives often requiring tact, diplomacy, empathy and negotiation skills and manage hostile and emotive situations through utilising well developed interpersonal skills.

Use a range of communication styles and channels as appropriate to the task.

Physical demands of the job

Frequent requirement to kneel, bend, lift, physically restrain and carry out movements in a confined space.

Frequent requirement to carry out physical interventions, as well as training staff in these interventions, e.g. medication administration including injections, ECT, use of sphygmomanometer, oxygen saturation.

The post holder will be exposed to collection and handling of bodily fluids and fouled linen.

The post holder will frequently be exposed to situations of violence and aggression which may require physical restraint.
