Do you have an interest in improving patient experience and a passion for implementing safe and timely hospital discharges for patients? Do you want to gain some invaluable experience in this important aspect of inpatient care? Then Trauma related services would like to hear from you.

An exciting opportunity has arisen for a Band 3 Discharge Co-ordinator, based at the LGI to join our existing Discharge team in trauma related services. The successful candidates will need to offer a proactive and responsive approach to coordinating timely discharges from hospital to home. You will play an important role within the Leeds Teaching Hospitals Trust in supporting the ward clinical staff to facilitate safe and timely discharge from hospital for our patients.

Main duties of the job

The successful candidate will require excellent communication skills to enable collaborative working with the multidisciplinary team across the CSU. The post holder must also be able to demonstrate excellent problem solving skills, work well under pressure and demonstrate the qualities underpinned by the Leeds Way.

The role of a discharge co-ordinator includes planning discharge from admission this includes communicating with patients and their relatives, involving them in their discharge decisions and making sure the patient is ready for their planned date of discharge and ensuring the team know the discharge plans.

We cover a seven day services 8am until 6pm and you will be expected to work some weekends and bank holidays as per rota.

JOB PURPOSE/SUMMARY

Working between two or more clinical areas the post holder will facilitate patient discharge providing a high-quality service in line with Trust standards. They will be able to communicate sensitivity and effectively with a wide range of people including patients, families, colleagues and agencies external to the Trust.

The Discharge Co-ordinator will manage patient flows within a defined area and will be responsible for prioritising work, reviewing working practices to ensure timely and efficient patient access to the Trusts services, delivery of the patient administration pathway from the decision to admit the patient and throughout the inpatient process.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

General

To provide a high standard of confidential, effective and accurate services that facilitates effective and efficient discharge.

Communicate respectfully sensitively and effectively with patients, relatives, colleagues and all other healthcare professionals.

Respect the working environment, to be responsible for ones own acts and omissions in relation to Health & Safety.

Respect confidentiality in line with national, Trust and local policies and guidelines.

The post-holder is expected to be honest, fair and consistent in their approach and to accept responsibility for ones own achievements, acts and omissions.

The post-holder will be expected to be a good team player, with the ability to work flexibly in a diverse and demanding environment to meet the evolving needs of patients and the department.

Support and guide colleagues who have less experience or during their induction period.

Responsible for appropriate use of equipment within the working environment.

A willingness to undergo all training requirements necessary for the post.

Undertake duties on other wards or departments as and when required by service demands or patients need, recognising and working within their own clinical limitations.

Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to the LTHT safeguarding policies.

Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes.

Complete mandatory training.

Service Delivery

Working between a minimum of two clinical areas facilitate seamless timely discharge or transfer of patients, within an agreed discharge plan.

Work closely with clinical teams and external agencies to support effective patient discharge.

Promptly alerts the Nurse in Charge of any delays or difficulties within a patients discharge plan.

Promptly alerts Nurse in charge to difficulties in patient flow, placement or when capacity issues arise within their responsible ward areas.

Collects and maintains accurate and up to date information regarding patient flows (acute and elective admissions, discharges, transfers, delays).

Ensure all patient and carer enquiries are dealt with in a friendly and professional manner.

Participates in MDT meetings to ensure seamless services are delivered.
