An exciting opportunity has arisen for a Band 3 Discharge Co-ordinator, based at the LGI to join our existing Discharge team in trauma related services. The successful candidates will need to offer a proactive and responsive approach to coordinating timely discharges from hospital to home. You will play an important role within the Leeds Teaching Hospitals Trust in supporting the ward clinical staff to facilitate safe and timely discharge from hospital for our patients.
Main duties of the job
The successful candidate will require excellent communication skills to enable collaborative working with the multidisciplinary team across the CSU. The post holder must also be able to demonstrate excellent problem solving skills, work well under pressure and demonstrate the qualities underpinned by the Leeds Way.
The role of a discharge co-ordinator includes planning discharge from admission this includes communicating with patients and their relatives, involving them in their discharge decisions and making sure the patient is ready for their planned date of discharge and ensuring the team know the discharge plans.
We cover a seven day services 8am until 6pm and you will be expected to work some weekends and bank holidays as per rota.
