J21 accepts patients aged over 75 directly from the Emergency Department and is responsible for commencing the initial treatment and care required. Patients will normally stay on J21 for a period of no more than a few days before they are either discharged or transferred to a longer stay ward. This high patient turnover makes J21 a busy and fast paced unit, inhabited by a hard working and supportive team that pride themselves in working collaboratively to provide high quality patient care..

J21 is immensely proud of its standard of quality care for all our patients, and we are looking for a highly skilled Sister/Charge Nurse nurse to join our team in caring for our older adult population in Leeds. We are looking for an enthusiastic and dynamic individual with the capability to empower and lead a team whilst remaining patient centered, co-ordinate clinical shifts, and support the Band 7 with the management of staff, and the implementation of continuous positive development.

Main duties of the job

Acts in accordance with the Nursing & Midwifery Council (NMC) Code of Conduct.

Communicate with patients and carers to ensure they have access to appropriate information.

Maintain and deliver compassionate patient focused care in accordance with LTHT guidelines, policies, values and behaviours.

Provide assessment, planning, implementation and evaluation of nursing care for patients, ensuring documentation standards are upheld in accordance with NMC and LTHT guidelines, whether written or computerised.

Promote and maintain positive relationships between all staff involved in the care of patients.

Promote evidence based practice.

Participate in the audit and evaluation of clinical practice in the interests of promoting and improving quality care through action on findings.

Undertake duties on other wards or departments as and when required by service demands or patient need.

Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies.

Contribute to the promotion of a positive clinical learning environment.

Keep up to date with developments, advances and research in individualised patient care.

About us

J21 is a diverse, hardworking and supportive team within the Speciality and Integrated Medicine (SIM) CSU, and are passionate about providing high quality, patient centred care to their patients. As an Acute Elderly Admissions Ward, J21 accepts patients over the age of 75 directly from the Emergency Department and so is responsible for, not only commencing their required treatment, but ensuring patients have the best possible start to their hospital admission. The comprehensive nursing admissions that are carried out at the start of the patient journey help to form the foundations for patients care during their stay, and so the nursing and care support worker team work hard to ensure these assessments are thorough and accurate, and that patients feel safe and well cared for. The J21 team pride themselves in being an inclusive and supportive team that will provide a welcoming and supportive environment for new starters
