Job summary

There is an exciting opportunity to work within the Specialty and Integrated Medicine CSU as a Discharge Coordinator. This involves working between one or more clinical areas to facilitate patient discharges, providing a high quality service in line with Trust standards. They will be able to communicate sensitively and effectively with a wide range of people including patients, families, colleagues and agencies external to the Trust.

Main duties of the job

The discharge co-ordinator will facilitate timely, safe, and effective patient discharge through close collaboration with multidisciplinary teams and external agencies. Participate in board rounds and MDT meetings to ensure clear, accurate discharge plans documented in PPM+. Communicate sensitively with patients, families, and professionals to provide up-to-date information on discharge pathways. Monitor patient flow, identify delays, and escalate issues promptly to support seamless transitions of care. Maintain accurate records of discharge status, waiting times, and reasons for delay, ensuring compliance with Trust policies. Promote teamwork, guide less experienced colleagues, and uphold confidentiality, safeguarding, and health & safety standards while adapting flexibly to service demands

We are a diverse team working within the Specialty and Integrated Medicine CSU. Our discharge coordinators provide discharge coverage for all inpatient wards within the CSU including Elderly medicine, General medicine, and Infectious Diseases.

  • Significant experience working within a health care environment.
  • Knowledge and understanding of patient discharge pathways

Qualifications

  • 5 GCSEs grade A-C (including English and Mathematics (or equivalent) OR Level 3 Diploma/NVQ/BTEC (or equivalent).
  • A basic standard of computer literacy.
  • A willingness to undertake further training as and when required.

Desirable

  • Ability to work within data bases
