Job summary

We currently have a number of exciting opportunities for the appointment of dynamic, self-motivated Band 6 Patient Flow Coordinators to work as part of the Patient Flow Coordinator Team within Specialty and Integrated as well as Acute Medicine based at St Jamess University Hospital Leeds.

We are looking for enthusiastic individuals to take responsibility and actively manage patient flow across the Clinical Service Units by implementing and maintaining processes that safely expedite patient placement and discharge.

The jobholder is responsible for the timely and appropriate placement of patients requiring admission to the Trusts bed base and for the escalation procedure from the Emergency Department.

The role will give you an insight into the wider organization and the issues and drivers which shape current NHS services. You will develop advanced skills in identifying risks and solving complex issues, and gain valuable experience in balancing staff availability and skills against the needs of patients across the service.

Main duties of the job

You will be required:

Support Team Lead in providing leadership to the clinical team

Support and inspire staff to provide efficient, effective, safe, quality care to patients

Deliver a positive patient experience

Promote the building of effective teams and collaboration between teams

Anticipate problems and take preventative or mitigating actions, or have contingency plans available

Assist in the line management of the nursing team, including individual and team performance, attendance, management and workforce planning

You should have knowledge in the field of hospital discharge and an understanding of the requirements of the national hospital discharge and community support policy and operating model.

Job responsibilities

The jobholder will play an essential role assisting the Team Lead with the 24 hour continuing responsibility for the effective and efficient management of the department. They will contribute to the provision of professional, clinical, managerial leadership and supervision for the department. The jobholder is responsible for helping to develop and maintain the departments philosophy that has the patient at the centre of compassionate safe and quality care delivery. By assisting with managing the administrative and financial aspects of the department, the jobholder will support the cost effective use of resources.

As an experienced nurse the jobholder will assist in co-ordinating activities on the department ensuring effective communication is maintained at all times, providing support to qualified and unqualified nurses within the department. He/she will play a key role in the assessment and placement of patients requiring admission to the Trusts bed base.

The jobholder will act as a role model, promoting quality evidence-based practice, developing the team and ensuring effective management of the department in the absence of the Team Lead.

Work efficiently to facilitate the quick movement of patients from the emergency department into the medical bed base. The jobholder will need to be competent in multitasking management and be able to make evidence-based decisions. The jobholder needs to work well in high pressured situations and be confident to communicate with all levels of the multidisciplinary team.

The jobholder will communicate the Clinical Service Units position to the senior teams and escalate any issues that need resolving.

Qualifications

Recognised teaching/assessing qualification (desirable)
