We
are offering an exciting opportunity to join the Band 6 workforce on J34
within the Speciality and Integrated Medicine Clinical Service Unit (CSU). We
are looking for enthusiastic registered nurses to join our friendly,
developing senior nursing team.J34
is a 28 bedded ward, specialising in Older Adult Care for patients who are
Medically Optimised for Discharge, accommodating both male and female
patients. Successful candidates will be responsible
for leading the staff and working in close collaboration with the MDT. The
role is to provide and be responsible for the assessment, planning,
implementation, evaluation of individualised programmes of care and to work
with the discharge team to facilitate safe discharge of patient. You
will deliver care without supervision and take responsibility as the prime
care provider for a group of patients and at all times promote confidence in
our commitment to provide excellent compassionate patient care. A
passion for working with our patients, strong leadership and performance
management is an integral aspect of this band 6 role with patient experience
and patient safety underpinning all aspects of the ward running efficiently
and effectively. You
will be responsible for the nursing assessments as well as the operational
running of the ward and all it encompasses. You
will need to provide assurance that standards of care are met and sustained
at all times.

with patients and carers to ensure they have access to appropriate

Maintain
and deliver compassionate patient focused care in accordance with LTHT
guidelines, policies, values and behaviours. Provide assessment, planning,
implementation and evaluation of nursing care for patients, ensuring
documentation standards are upheld in accordance with NMC and LTHT
guidelines, whether written or computerised.
Promote and maintain positive relationships
between all staff involved in the care of patients.
Promote
evidence-based practice.
Participate
in the audit and evaluation of clinical practice in the interests of
promoting and improving quality care through action on findings.
Undertake duties on other wards or
departments as and when required by service demands or patient need.
Act in such a way that always safeguards the
health and wellbeing of children and vulnerable adults. Is familiar with and
adheres to, the LTHT safeguarding policies.
Contribute to the promotion of a positive
clinical learning environment.
Keep up to date with developments, advances
and research in individualised patient care.Participate in research and or education
programmes relating to professional nursing issues and practice.In
the absence of the Senior Charge Nurse take charge of the ward or department
and co-ordinate ward or department activities.
