Job responsibilities
The
post holder is responsible for the management of a defined caseload of
patients. The post holder will be
responsible for identifying, assessing and recommending / initiating
treatment for patients who have suffered a fragility fracture in patients
over the age of 50; with the aim of reducing their risk of subsequent or
secondary fractures. The post holder will also contribute to the initial
development of the Fracture Liaison Service and the on-going subsequent
improvement of the Service in LTHT and wider, using resource cost
effectively.
The
post holder will be expected to request DXA scans following appropriate
training as part of their role and will be expected to complete the nurse
prescribing course whilst in the post.
The
post holder will be responsible for the delivery of the clinical pathway.
Assess health needs of individual
patients and provide care to meet these needs through a process of shared
decision making. Accept referrals to
agreed criteria and work in partnership with other professionals to deliver
co-ordinated programmes of care effectively and referring patients to other
practitioners as required.
Deliver expert clinical care to
patients in agreed practice settings, working as part of
nursing/multidisciplinary teams.
Communicate with patients in ways
that empower them to make informed choices about their health and health
care.
Provide appropriate education to
patients and their families to promote health and encourage self-care.
Use communication skills to impart
information to patients and carers and provide them with advice and emotional
support as appropriate.
Maintain adequate patient
documentation to up-load onto the Fracture Liaison Service Database.
Be involved with developing
standards of care, practice guidelines and where appropriate Care Pathways
for the patient/client group.
Demonstrate and share clinical
practice developments to provide evidence-based care and contribute to
service improvement in accordance with local policy initiatives.
Contribute appropriately to
clinical governance activities that relate to own area of practice and
patient/client group.
Support clinical and management
teams in the delivery of Trust and Clinical Service Unit objectives.
Effective prioritisation of
workload and time management when lone working or as part of team.

appropriate) to ensure seamless provision of care delivery

- Provide training and education of
learners and other junior staff

patients and their carers.

accidents, complaints, untoward incidents and other significant events

the development of nurses as appropriate

participates in or leads the development of specific elements of nursing
practice, reflecting the CMT and Nursing Directorate business plans
- Participates in the setting,
monitoring and improvement of standards and the quality of patient care,
including those standards defined in the National Benchmarks for Essential
Care
- Promote evidence-based practice,
wherever possible
- Engages users and carers views on
service delivery and development
- Implement policy and practice guidelines
specific to the clinical specialty, proposing changes where appropriate
- Contribute to policy and protocol
development for speciality area

in relation to equipment, resource and stock control utilising equipment and
supplies appropriately.
