Job summary

Do you want to be part of an exciting and inspirational team? That will support, develop and help manage the delivery of patient centred care within Leeds Teaching Hospitals Bone Health Service. Within Endocrinology we are looking for two Band 6 Specialist Nurses who are enthusiastic and highly motived to work as part of a dynamic team. The Bone Health multidisciplinary service, aims to identify, investigate and initiate treatment. We work with local healthcare partners to deliver a multi-agency holistic service for people over the age of 50 within the local area who have suffered a fragility fracture, with the aim of reducing their risk of subsequent fractures. The successful applicants will be responsible for identifying these patients, and in conjunction with other members of the healthcare team, co-ordinate, provide advice, support and education to the patient in relation to on-going treatment and prevention of secondary fractures. The specialist practitioner will contribute to the care of the patient group by leading on defined aspects of care and communicate with patients in ways that empower them to make informed choices about their health and healthcare. Main duties of the job

Be educated to nursing degree level

Have a minimum of 1 year post registration nursing experience

The Endocrinology Department at Leeds Teaching Hospitals is a Multi-Disciplinary Team that delivers a high quality service for patients attending for pituitary and adrenal dynamic testing and osteoporosis treatments. The team of Specialist Nurses are dedicated to the development of Endocrinology nursing and passionate about the delivery of high quality patient care. Job responsibilities

The post holder is responsible for the management of a defined caseload of patients. The post holder will be responsible for identifying, assessing and recommending / initiating treatment for patients who have suffered a fragility fracture in patients over the age of 50; with the aim of reducing their risk of subsequent or secondary fractures. The post holder will also contribute to the initial development of the Fracture Liaison Service and the on-going subsequent improvement of the Service in LTHT and wider, using resource cost effectively.

The post holder will be expected to request DXA scans following appropriate training as part of their role and will be expected to complete the nurse prescribing course whilst in the post.

The post holder will be responsible for the delivery of the clinical pathway.

Assess health needs of individual patients and provide care to meet these needs through a process of shared decision making. Accept referrals to agreed criteria and work in partnership with other professionals to deliver co-ordinated programmes of care effectively and referring patients to other practitioners as required.

Deliver expert clinical care to patients in agreed practice settings, working as part of nursing/multidisciplinary teams.

Communicate with patients in ways that empower them to make informed choices about their health and health care.

Provide appropriate education to patients and their families to promote health and encourage self-care.

Use communication skills to impart information to patients and carers and provide them with advice and emotional support as appropriate.

Maintain adequate patient documentation to up-load onto the Fracture Liaison Service Database.
