Job summary

An exciting opportunity has arisen for a patient centered individual to join the Radiology Admin CSU for the role of Cancer Pathway Manager to support the delivery of the Prostate Fast Track Service.

The Cancer Pathway Manager provides a pivotal role in ensuring the timely and efficient coordination of care for patients referred with suspected or confirmed prostate cancer. Working closely with clinical teams across radiology, urology and pathology, administrative staff, and multidisciplinary team (MDT) members, the post-holder supports the delivery of national cancer targets, facilitates smooth patient journeys, and ensures robust tracking of diagnostics, staging, and treatment planning processes. Specifically, this role is integral to the implementation of the RAPID prostate cancer pathway at LTHT, and the evaluation of AI in supporting this pathway.

There will be elements of pathway management and some supervisory duties that would make this a varied role where no day would be the same. There will also be data collection and analysis with regard to breach dates but full training and support will be given. This is a patient-facing role that requires excellent communication, strong organisational skills, and a detailed understanding of the prostate cancer diagnostic and treatment pathway.

Main duties of the job

The post-holder will coordinate, proactively manage and track prostate patients against the National Cancer Waiting Times Standards. They will escalate problems to Patient Service Coordinators, Business Managers and Clinicians in a timely manner so that remedial action can be taken where necessary to ensure the Cancer Waiting Times targets are met.

The post-holder will coordinate all aspects of the patient pathway in their specialty and be responsible for prioritising work, reviewing work practices.

The post holder will work closely with the project team to support the implementation of the RAPID cancer pathway at LTHT. This will involve patient facing contact along with data collection.

Job responsibilities

JOB PURPOSE

The Radiology & MIS Clinical Service Unit (CSU) at LTHT provides a range of diagnostic imaging, including fast track imaging for patients who are suspected of having cancer. This role has been created to support the CSU in its delivery of diagnostics for patients on a prostate cancer pathway, ensuring diagnostic imaging is provided in line with local and national targets. As this is a new role within the Radiology CSU, the post holder will need to be flexible to support the needs of CSU as it evolves over time. The Cancer Pathway Manager will provide comprehensive support to the Radiology CSU Management Team through the co-ordination and management of patients on a cancer pathway that require diagnostic imaging.

The post-holder act as a single point of contact for the CSU, and as a liaison point between other CSUs, Multi-disciplinary Teams (MDTs), and the Corporate Cancer Team.

1. Pathway Coordination

Act as the key contact for patients undergoing investigation or treatment for prostate cancer.

Coordinate appointments, diagnostics (e.g. MRI, CT scan, biopsy), and referrals to ensure pathway milestones are met within nationally mandated timescales (e.g. 28-day diagnosis, 62-day treatment target).

Track and monitor patients through the prostate cancer pathway using relevant IT systems and databases.

2. MDT Coordination

Support the weekly urology/prostate MDT by ensuring seamless patient flow through the pathway, chasing outstanding diagnostics, and ensuring accurate data entry.

Ensure actions from MDT meetings are completed and communicated to the relevant professionals in a timely manner.

3. Communication and Liaison

Liaise effectively with urologists, radiologists, specialist nurses, administrative teams and patients to provide a seamless service

Provide empathetic support to patients and families, ensuring they are well-informed and signposted to appropriate services as needed.

4. Data Management and Reporting

Maintain accurate and timely records on cancer tracking systems.

Assist in the collection and reporting of data for audit, performance monitoring, and cancer performance returns.

Support the validation of waiting time breaches and help identify bottlenecks in the pathway.

5. Service Improvement

Identify inefficiencies and escalate issues to senior colleagues to support ongoing pathway optimisation.

Contribute to quality improvement initiatives and service evaluations related to the prostate cancer pathway.

Work seamlessly as part of the project implementation team on the RAPID prostate cancer pathway.
