An
exciting opportunity has arisen for a dynamic, motivated experienced
registered nurse to join the anticoagulation and thrombosis service as a
trust wide band 6 VTE Clinical Nurse specialist.
The
purpose of the Venous Thrombombolism (VTE Clinical Nurse Specialist (CNS)
role is to ensure high quality VTE prevention care is embedded throughout the
organisation alongside the lead VTE prevention nurse, with the aim of
reducing preventable patient harm from hospital associated thrombosis. In
addition, the Post holder will work as a clinical nurse specialist to lead
and deliver safe and high-quality nurse-led inpatient VTE care, and support
and deliver VTE follow up clinics as a member of the multi-disciplinary
thrombosis and anticoagulation team alongside the advanced practitioner for
VTE.
The
nurse specialist will take responsibility for leading on VTE prevention care,
and ensuring adherence to the NICE VTE guidance, to include surveillance,
analysis and improvement of VTE risk assessment rates, hospital associated
thrombosis cases, VTE staff education, VTE patient information and
appropriate use of thromboprophylaxis measures. Activity will involve data
collection, analysis, presentation and dissemination, staff education
activities, awareness initiatives and quality improvement work.
Main duties of the job
Triage
referrals to the VTE team.
Review
VTE inpatients - counsel on VTE diagnosis and treatment and provide patient
information.
Ensure
appropriate follow up is arranged as per NICE recommendations.
Support/deliver
VTE follow up clinics within scope of practice
To
attend VTE MDT, ensuring all patients are presented.
To
develop expert knowledge around VTE prevention and thrombosis management
To
work autonomously as an expert practitioner in the nurse led anticoagulation
and thrombosis services providing safe and effective patient centred care.
To
contribute to audit and research activities within the speciality and in
collaboration with multi-disciplinary colleagues
To
participate in the identification and development of VTE guidelines/clinical
protocols and strategies to enhance both the continuity and standard of
specialist care whilst ensuring equity of access to the service.
Identify
clinical issues and incidents within the Trust that reduce the quality of
care within the specialist service.
To
use initiative and build strong working relationships to positively influence
VTE prevention practice across the Trust.
To
actively support clinical staff to provide high quality VTE prevention care
To
contribute positively to the leadership of nursing within the care group
acting as an effective role model.
To involve service users in providing feedback of their experience of
the current service and suggestions for improvements.
