This Collaborative Working Project aims to optimise workforce capacity within the early breast cancer and advanced breast cancer oncology services. The post will work alongside a Band 7 Clinical Nurse Specialist (CNS) or Band 7 Advanced Clinical Practitioner (ACP), and the wider Non-surgical Oncology clinical and administrative workforce.

Through the implementation of this CWP, the aim is to achieve the following:

Safeguard the future of the Breast Cancer Service at Leeds Teaching Hospital.

Create a map of the early breast cancer and advanced breast cancer oncology services using established Process Mapping Methodologies to conduct a pathway mapping exercise, identify potential areas for improvement and integration of the new roles into the relevant aspects of the service.

Map the care pathways across early breast cancer and advanced breast cancer oncology; redesigning them in a cohesive way to engage and upskill non-medical prescribers in a most efficient way

Ensure early breast cancer and advanced breast cancer oncology patients are seen, reviewed and treated in an efficient and timely manner.

Establish a protocol for the stratification of patients into specific clinics manually or through the support of an online symptom monitoring system using ePROMs (electronic Patient Reported Outcomes Measures) for selected stable patient populations to monitor treatments and improve Patient-Initiated Follow-Up.

Identify patient treatment pathways suitable for ePROMs and implement a minimum of one pathway to evaluate impact through audit and data collection with a view to implementing the full complement of appropriate therapy pathways.

Specifically, develop ePROMs-supported protocols and pathways in early breast cancer, pilot them in the service in Leeds and share the protocols with Non-Surgical Oncology service reconfiguration programme to provide a standardised approach across the region.

Reduce unwarranted variation in care quality and implement a cohesive treatment pathway in the North of the WYYAT Group through the development of protocols that can be shared and adopted throughout North WYAAT, utilising this CWP as an exemplar for WYAAT.

The post holder will be responsible for data collection and database entry across all aspects of the collaborative working project. The post holder will work closely with the clinical lead for the project to collect data and will maintain database activity and analyse data as required.

The post holder must be able to communicate sensitively and effectively with a wide range of people including medical and nursing staff and the wider multi-disciplinary team with possible patient interaction.
