Job summary

We are looking to recruit an experienced hospital based palliative care specialist nurse to join our Palliative Care Team, to lead the development and implementation of our education programme.

You must be an experienced adult specialist palliative care clinical nurse specialist , with expert knowledge across all disease groups. The postholder will be responsible for providing advanced nursing expertise to patients and be responsible for providing complex assessments in a range of circumstances where excellent interpersonal and communication skills are essential.

Working alongside nursing and medical colleagues in the team you will strategically plan, deliver and evaluate high quality education and development initiatives. This pivotal role is also central to supporting the ongoing development of the CNSs in the team, to ensure our nursing team are equipped with the specialist skills, confidence and knowledge to deliver outstanding care to our patients and their families.

Main duties of the job

The postholder will work in a large hospital based multidisciplinary team delivering a responsive service to patients with specialist palliative care needs.

The team provide a 7 day face to face service and the role involves supporting the delivery of the weekend service. The role will work closely with specialist providers across the region and with multidisciplinary teams in the Hospital to ensure our patients receive excellent holistic care meeting their individualised needs. You will act as an expert resource and be a positive role model providing sound professional knowledge and leadership to all members of the multidisciplinary team.

You will work in a blended role with clinical care and non clinical responsibilities for an extensive education portfolio. You will be an enthusiastic and innovative educator with established skills of not only delivering education but also the development of programmes with the ability to adapt to the changing landscape within clinical areas.

About us

Leeds Teaching Hospitals is one of the largest NHS Trusts in the country, offering a wide range of specialist and general hospital services. The Palliative Care Team consists of a team of clinical nurse specialist in palliative and end of life care, consultants in palliative medicine, speciality grade doctors, a pharmacist and an admin team. We also have placements for training doctors and student nurses.

We have an excellent reputation across the Trust and regionally / nationally for our responsive service, end of life care programme and extensive education portfolio.

The service responds to approximately 2000 referrals per year and provides education to the wider workforce delivering core palliative care and contributes to education and training at pre and post qualifying levels in medicine, nursing and allied health professionals.

The Jobholder will carry continuing responsibility for a defined caseload, including the setting of standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care. The jobholder will provide advanced nursing expertise to a specific patient group and will contribute to the development of the speciality service as well as contributing to the strategic direction of nursing within the Trust. The jobholder will ensure cost effective use of resources.

The post holder is responsible for the management of a defined caseload of patients with specialist palliative care needs. The post holder will carry out the assessment of those needs, working with the treating teams in the planning, implementation and evaluation of measures to support both the patient and those close to them

The post holder will provide advanced nursing expertise to a specific patient group and will also contribute to the development of the palliative care service in LTHT and wider, using resources cost effectively, as well as contributing to the strategic direction of nursing within the Trust.

The jobholder will manage and lead a team of nurse specialists at Band 6 and below and be responsible for the effective use of resources within the clinical team.

Working in partnership with Matron, Consultants in Palliative Medicine and SPC Team Leader, to lead on the development and implementation of a robust and innovative education strategy for all staff across the organisation in line with the national Strategy for End of Life Care (Ambitions for Palliative and End of Life Care) and locally agreed key quality improvements (to be implemented) within the LTHT. This includes education on quality initiatives such as: advance care planning, Electronic palliative care coordinating systems, Treatment escalation plans, Rapid discharge, Priorities of care the dying person. Processes should be developed to measure the effectiveness of any education initiatives.

The post holder will also hold responsibility of leading the development of the band 6 CNS team.
