Job summary
An exciting opportunity has arisen for a highly motivated, dynamic and experienced nurse to join our Anticoagulation Clinical Nurse Specialist Team in Bexley Wing at St. Jamess Hospital in Leeds as a band 7 Team Leader.
The Anticoagulation team is responsible for the management of approx. 6,000 patients on anticoagulant therapy across Leeds area. The clinics are based on both St James site and in hubs across the city.
As team leader you will have responsibility for leading, supporting and managing a group of clinical nurse specialists and clinical support workers. The role involves establishing clear lines of communication with the multidisciplinary teams, providing advice, support and education where appropriate and to ensure cohesive multidisciplinary management of patients. The successful candidate will be committed to the development and delivery of this specialised service and will have excellent communication skills, evidence of service development and strong leadership skills.
Main duties of the job
Working within the multidisciplinary Anti-coagulation service (ACS), you will lead a team of clinical nurse specialists and support workers. You will act as a clinical role model providing advice, support and monitoring for patients on Warfarin, LMWH, and DOACs for various conditions such as AF, Heart Valves, PE, DVT, Budd Chari syndrome and Antiphospolipid syndrome.
The ACS team are based at St James Hospital but also provide clinics at Wharfedale, Seacroft and Chapel Allerton Hospitals; host community-based clinics and provide a home visit service. The team also triage referrals, administer medications, advise patients on correct dose of warfarin based on INR results, perform venepuncture and provide health education.
The role will require the successful candidate to travel across the Leeds area. A full driving licence is essential. The team have use of 3 lease cars for travel to & from home visits and clinics.
