Job summary
Leeds Teaching Hospitals have been successful in achieving funding for a band 7 Clinical Nurse Specialist in Adult Anti-Phospholipid Syndrome (APS) from APS Support UK. We seek to employ a full time Band 7 Clinical Nurse Specialist for a one-year secondment into this new role for our Trust.
This is the first role of this type in the region and the Thrombosis team are committed to supporting the development of a dedicated experienced and knowledgeable CNS to provide a point of contact for queries or concerns that arise outside the setting of the clinic consultation for our patients with APS. The successful applicant would be involved in redesigning current clinics which are currently set up to encompass all complex thrombosis as our cohort of patients living with APS are not presently prioritised effectively. The APS Nurse Specialist would, with initial supervision, run dedicated APS clinics.
The vision of this post it to work with the MDT to provide practical and emotional support, counselling, literature, and other resources to patients with APS, supporting them to come to terms and living with this complex and often multisystem potentially life-threatening condition. The successful candidate would lead a regional programme of education to both primary and secondary care in the role of appropriate testing for APS, supporting anticoagulation and management of complications.
Main duties of the job
Activity will be recorded, including number of consultant led clinic appointments attended/supported, number of inpatient visits, nurse led clinics held.
Offer telephone reviews for those with challenges to attend in person (success to be reflected by patient satisfaction survey, reduced did not attend rate)
Successful schedule of surveillance investigations e.g. Echocardiograms and Urinary PCRs (spreadsheet of competed investigations and frequency for each patient) and onwards referrals to cardiology or renal teams as appropriate.
Implementation of a patient survey of experience of both existing patients follow up and new patient diagnoses. This would include experience of patients opting for telephone consultations or outreach clinics.
Provide a non-urgent contact line Monday Friday 9am - 5pm for patients with APS and colleagues with APS related queries. Due to the part time nature of this proposed post (see later) we envisage that all messages left should be addressed by the next working day which will require cross cover arrangement with existing team members during periods of leave.
